The German E. coli outbreak: now it’s sprouts?

I haven’t said anything about the E. coli 0104 crisis in Germany up to now because I’ve been waiting for the evidence.  Without evidence, the source of the outbreak remains uncertain.

Yesterday, the German minister of agriculture announced that sprouts are the cause.  But are they?

What  is known without question is that the outbreak is deadly serious.  Bill Marler reports these shocking numbers as of June 5:

Deaths = 22 (21 in Germany, 1 in Sweden)

Illnesses = 2,243 (2,153 in Germany, and 90 more in 10 other European nations and the U.S.)

Cases of Hemolytic Uremia Syndrome (HUS) = 627

Why shocking?  This is a devastating disease, excruciatingly painful, with a high probability of causing lifelong complications.  And the disease is almost entirely preventable by following standard food safety procedures.

The idea that the cause is sprouts, and German sprouts at that, comes as a surprise.  Why?  First, sprouts are a frequent cause of foodborne illness and should have been high on the list of suspected foods.  Second, sprouts did not turn up in the case-control studies.

Instead, investigators examined cucumbers, lettuce, and tomatoes (and, in the process, put Spanish cucumber producers out of business).  As Marler explains, the German authorities didn’t want to take a chance, given the results of their investigation.

The case-control investigation was conducted by the Robert Koch Institute, the German equivalent of our CDC.

  • The cases: From May 29 to June 2, investigators interviewed 46 affected patients from Bremen, Hamburg and Lübeck about the foods they had eaten.
  • The controls: They interviewed 2,100 people who were not sick but were of similar age group, sex and region of residence.
  • The results:

95% of the Cases had eaten at least one of the three vegetables.

This evidence strongly implicates these vegetables.  But did they not look for sprouts?

In another related study of people from a Frankfurt business company who had become ill, those who had eaten from the salad bar in the company cafeteria had a 7-fold increased risk of developing bloody diarrhea than those who had not.  No such association was seen for other foods investigated, such as dessert, fruit and asparagus.  Sprouts are not mentioned.  How come?

In trying to figure out what’s going on here, a BBC World News report raises even more questions (my emphasis):

The agriculture minister for Lower Saxony, Gert Lindemann, said there was a clear trail of evidence pointing to a plant nursery south of Hamburg [as the source of the contaminated sprouts].

The nursery has been closed, though officials say the outbreak’s source cannot yet be definitively confirmed.

…Mr Lindemann said epidemiological studies all seemed to point to the plant nursery in Uelzen in the state of Lower Saxony, about 100km (62m) south of Hamburg – though official tests had not yet shown the presence of the bacteria there.

“Further evidence has emerged which points to a plant nursery in Uelzen as the source of the EHEC cases, or at least one of the sources,” he said.  [What evidence?]

…Gert Hahne, a spokesman for the Lower Saxony agriculture ministry, earlier told the Associated Press news agency that many restaurants in which people ate before becoming ill had recently taken delivery of the sprouts. [Guilt by association]

He said authorities would still maintain a warning against eating tomatoes, cucumbers or lettuce.

The health ministry in Berlin said it was still waiting for results from tests on the beansprouts, Germany’s DPA news agency reported.

And the head of the Robert Koch Institute (RKI), Germany’s national disease centre, was also reported as saying that the cause of the outbreak could not yet be confirmed.

So: are sprouts the cause?

By this time, the outbreak is slowing down as the contaminated foods make their way through the food supply.

Could this happen here?  You bet.

If ever there was a time to give the FDA more resources, now is it.  The FDA now has the authority to impose standard food safety procedures on food producers and to require safety measures for the foods we import.   But Congress wants to cut the agency’s budget, and badly.

Now would be a good time to let congressional representatives know that we need a stronger FDA.   And while you are at it, let the USDA know that you think it would be a good idea to regulate other forms of toxic E. coli as adulterants in the same way they regulate E. coli 0157:H7. There is plenty government could do right now to protect us from outbreaks like this one.

A word about sprouts:

How come sprouts are such frequent sources of food safety problems?

Sprouts are grown from tiny seeds that are impossible to wash thoroughly enough to ensure that they are free of harmful bacteria.  The seeds are sprouted in water that must be changed several times a day.  This water is an excellent growth medium for bacteria.  That is why FDA guidance says sprout producers ought to test the wash water for harmful bacteria.

Under the new legislation, the FDA has the authority to enforce this guidance.  But does it have adequate personnel?  Unlikely, given the current stance in Congress.

This just in:  No, it’s not sprouts, according to this bulletin from Food Chemical News:

The latest news, reported this morning by both the Associated Press and BBC, is that 23 of 40 samples of organic sprouts taken from the Gaetnerhof farm in the Lower Saxony region of Germany have tested negative for the bacteria. Tests on the other samples have yet to be returned.

Everybody loves MyPlate. Really?

I’ve been collecting public reactions to MyPlate, not least because I’m quoted in many of them.

The USDA collected supportive blurbs from a wide ranging group of supporters (mine among them)

Weight Watchers saluted the new icon in a full-page ad in the Washington Post

The Los Angeles Times covered it (I’m quoted)

Marian Burros wrote about it for Rodale (I’m not quoted).  She points out:

First of all, the complexities of good nutrition haven’t disappeared. You can find some of them at ChooseMyPlate.gov. And there are factors contributing to obesity—the unhealthy practices of the food industry, the presence of obesogens in the environment, an unsustainable food system to name a few—that won’t go away just because the pyramid is gone.

The American Heart Association supports it.

The American Dietetic Association supports it:

As we have in past years, the American Dietetic Association was deeply involved in the development of the 2010 Dietary Guidelines. And we will use the Guidelines and the new MyPlate to provide the unequalled advice and services of registered dietitians to individuals and communities alike.

Unsurprisingly, the produce industry loves it, according to The Packer and the trade association, United Fresh.

The meat industry spins it (MeatPoultry.com):

We are pleased that the new food icon unveiled today…affirms in a clear and simple fashion that protein is a critical component of a balanced, healthy diet….Lean meat and poultry products are some the most nutrient rich foods available, are excellent sources of complete protein, iron and zinc and maintain an excellent nutrition per calorie ratio.

Amber Healy of Food Chemical News (the site is only open to subscribers) has collected a bunch of food industry responses.  Here are some excerpts:

Jeane Wharton, executive director of the U.S. Dry Bean Council, couldn’t be more delighted with Thursday’s news.  “Beans are a great protein, and they’re also a vegetable,” says Wharton, who observes that her industry’s product benefits twice from the new image.

The American Bakers Association joined in with their praise of the plate, especially for grains “appropriately occupying a large portion on the dinner plate,” which shows that the agencies responsible for nutrition advice “are making a strong statement regarding the importance of grains as the foundation of a healthy lifestyle.

The National Dairy Council, National Milk Producers Federation, International Dairy Foods Association and Milk Processor Education Program (MilkPEP) also praise the new image in a statement released Thursday. While dairy isn’t included on the plate directly, the importance of dairy products in a healthy diet is clear, they say.

“Dairy foods are rightfully being recognized — from the school house to the White House — as an important part of everyone’s diet,” says Jerry Kozak, NMPF’s president and CEO. “USDA’s new icon, with a simple visual metaphor of a serving of dairy products alongside a plate, says it’s vital to consume three servings of low-fat and fat-free dairy foods every day.” [Really?  Vital?  That’s not how I read it.]

Last but not least, Andy Bellatti in his Small Bites blog and Melanie Warner on BNET provide excellent analyses of the disconnect between MyPlate recommendations and current agricultural policies—both coming from the same, historically schizophrenic USDA.

Why I think health claims are about marketing, not health? Gerber’s whey claim

Yesterday’s New York Times displayed a full-page advertisement for Gerber baby food (owned by Nestlé, no relation):

Gerber is taking more than baby steps to reduce the risk of certain allergies

.…The FDA concludes that current scientific evidence is appropriate for consideration of a claim regarding the relationship between the consumption of 100% whey-protein partially hydrolyzed infant formula and reduced risk of atopic dermatitis.

….We’re proud to say that Gerber Good Start is the first and only formula brand made from 100% whey-protein partially hydrolyzed.  In contrast, most other routine milk-based formulas are made with intact cow’s milk protein.

Translation: Some infants are allergic to the proteins (whey) in cow’s milk.  Treating the proteins so they are split apart into smaller fragments (partial hydrolysis) apparently destroys some of their ability to elicit allergic immune reactions in the skin.

But here’s where the ad gets totally weird:

The FDA has concluded that the relationship between 100% whey-protein partially hydrolyzed infant formulas and the reduced risk of atopic dermatitis is uncertain, because there is little scientific evidence for the relationship.  Partially hydrolyzed formulas should not be fed to infants who are allergic to milk or to infants with existing milk allergy symptoms.

Huh?

Blame Congress for this one.  It insists that the FDA allow “qualified” health claims” for which scientific evidence is uncertain.

If you want to know why the FDA can’t seem to get anything done, take a look at what it’s staff had to do to respond to the Gerber petition.  Graduate students take note: this is an exhaustive review of scientific studies on the relationship between hydrolyzed whey protein and infant skin allergies.

Here is the FDA’s conclusion, written in FDA-speak, about Gerber’s petition for a health claim:

In light of the above considerations, FDA intends to consider the exercise of its enforcement discretion for the following qualified health claims (my emphasis):

1. “Very little scientific evidence suggests that, for healthy infants who are not exclusively breastfed and who have a family history of allergy, feeding a 100% Whey-Protein Partially Hydrolyzed infant formula from birth up to 4 months of age instead of a formula containing intact cow’s milk proteins may reduce the risk of developing atopic dermatitis throughout the 1st year of life and up to 3 years of age.”

2. “Little scientific evidence suggests that, for healthy infants who are not exclusively breastfed and who have a family history of allergy, feeding a 100% Whey-Protein Partially Hydrolyzed infant formula from birth up to 4 months of age instead of a formula containing intact cow’s milk proteins may reduce the risk of developing atopic dermatitis throughout the 1st year of life.”

3. “For healthy infants who are not exclusively breastfed and who have a family history of allergy, feeding a 100% Whey-Protein Partially Hydrolyzed infant formula from birth up to 4 months of age instead of a formula containing intact cow’s milk proteins may reduce the risk of developing atopic dermatitis throughout the 1st year of life and up to 3 years of age. Why would Gerber’s place a hugely expensive full-page ad in national newspapers to celebrate a decision like this?  Because it knows that any health claim, no matter how poorly substantiated by science, gives it a competitive advantage.

This is reason enough to promote breastfeeding.

Sugary drinks vs. obesity: power politics in action

It used to be that the “soda wars” referred to Coke vs. Pepsi.  No more.  Today’s soda wars are fought on the health front, as more and more evidence links sugary drinks to obesity and other health problems.

The current issue of the New Yorker has an article by John Seabrook (in which I am briefly quoted) about Pepsi’s attempt to “health up” its snacks and drinks.

Seabrook’s article, “Snacks for a fat planet,” describes the extraordinary amount of money and effort Pepsi is spending to try to tweak its products to make them seem healthier.  His article doesn’t exactly give Pepsi a pass (as some of my readers have complained), but it does not really come to grips with how sugary drinks and snacks affect health or how Pepsi is marketing its products in developing countries.

That, no doubt, is why Pepsi has sent out a press release to reports that enclosed the complete article and suggested that reporters might be “interested in the company’s focus on its innovative approach to:”

  • Reduce salt, fat and sugar across the portfolio – the New Yorker feature explains PepsiCo’s effort to re-shape natural salt so that it has more surface area, and, in turn, is perceived as “saltier” on the tongue – meaning they can maintain all the salty flavor in Lay’s but reduce overall sodium content
  • Scale more drinks and snacks made with whole grains, fruit, vegetables and dairy to new markets – e.g. bringing vegetable-based gazpacho (perhaps with an edible whole grain spoon) to the U.S.
  • Test new ingredients brought back from “treks” around the world – e.g. using a state-of-the-art robot in PepsiCo’s new Hawthorne, NY research lab to test botanicals and other natural ingredients from near and far – e.g. even secluded villages in the far East – to determine their impact on taste and viability for use in PepsiCo snacks and drinks (Do they intensify sweetness? Can they be a substitute for sugar? Do they have a particular healthful function?)

Score this one as a win for Pepsi.

Along with such pledges, Pepsi is aggressively marketing sodas to teenagers.  The San Francisco Chronicle reports on Pepsi’s new “social marketing” vending machines.

At a trade show in Chicago this week, PepsiCo rolled out a prototype interactive soda machine that lets you send a drink as a gift to a friend or a random stranger.

“Our vision is to use innovative technology to empower consumers and create new ways for them to engage with our brands, their social networks and each other at the point of purchase,” Mikel Durham, PepsiCo Foodservice’s chief innovation officer, said in a press release.

“Social Vending extends our consumers’ social networks beyond the confines of their own devices and transforms a static, transaction-oriented experience into something fun and exciting they’ll want to return to, again and again.”

But these kinds of marketing pushes are not confined to Pepsi.   Advertising Age reports that Joe Tripodi, Coke’s chief marketing officer of Coca-Cola explains the company’s growth strategy: focus on teenagers:

The company sees huge opportunities to grow colas, and the business as a whole, around the world in the next decade. Teen recruitment will be particularly important, as the company follows demographic trends.

“There was a time [a decade ago] when we walked away from teen recruitment and probably lost a generation of drinkers,” Mr. Tripodi said. “Parts of the world lost confidence in cola as the engine of growth. We’ve gotten that back in a big way. …When you look at the massive opportunity in so many huge countries in the world, that’s what energizes us and why we believe cola is still at its very early stages.”

And then there are partnership strategies. The latest is Sonic drive-ins’ campaign for Limeades for Learning. The campaign encourages eaters to vote for school projects like those that support physical activity.  Sonic promises to fund the projects with the most votes. The Limeades, by the way, are 620 calories (for a medium) or 950 calories (for a large).

Finally, for now, the Boston Globe reports that most Massachusetts voters support a sales tax on sodas if the money is used for some useful purpose.  But:

The American Beverage Association has been aggressively fighting taxes on soda, as cities and states across the country look for new tools to counter an obesity epidemic and raise revenue amid squeezed budgets. It has spent millions fighting initiatives that impose product-specific excise taxes on sugar-sweetened beverages and has been successful in nearly every attempt.

Expect more such public relations efforts superimposed on fundamental marketing techniques aimed at kids and fighting back on taxes and other attempts to limit soda intake.

 

 

 

How does this comport with the spanking new advertising guidelines to children or any of the previous pledges? Is sending a soda to a friend an activity or marketing? Or both?

Marion Nestle to Become Biggest New York McDonald’s Franchisee

Food and lifestyle critic Marion Nestle announced* this morning that she plans to invest in twelve Manhattan McDonald’s restaurants upon her retirement next month from New York University. The move will make the nutrition activist New York’s largest Golden Arches franchisee.

“It was a natural fit for me,” Nestle told The New York Times.* “After years of harping on the fast-food guys, I realized something shocking: People like affordable, tasty food. I’m certainly not going to get rich in my golden years by selling organic carrots and quinoa.” Former Times reporter Marion Burros returned from her own retirement to conduct the Marion-on-Marion interview.

A new special edition of Nestle’s book What to Eat is planned for the fall,* complete with a special cover designed by Hallmarks musical greeting-card department. Every time you open the book, Nestle’s own voice will be heard singing “Ba-da-bap-BAH-BAH! Im lovin it!”

Nestle added in a special Q&A for Mother Jones* that in her new role as a restaurateur, she would have to re-think practically everything she had written about food-industry marketing. “Momma’s got to make a living,” Nestle said. “I’ve promised the Socialist Scholars Conference that I’d co-sponsor next year’s event in Havana. So if I have to walk down Broadway dressed as Mayor McCheese to get butts in the seats, I’ll do it.”

Easiest way to cut calories: smaller packages!

In an effort to avoid having to raise prices and lose competitive advantage, the makers of processed foods are putting less food—and more air—in the packages.  The New York Times calls this “stealthy downsizing.”

In every economic downturn in the last few decades, companies have reduced the size of some products, disguising price increases and avoiding comparisons on same-size packages, before and after an increase. Each time, the marketing campaigns are coy; this time, the smaller versions are “greener” (packages good for the environment) or more “portable” (little carry bags for the takeout lifestyle) or “healthier” (fewer calories)….Most companies reduce products quietly, hoping consumers are not reading labels too closely.

I’d call it “healthy downsizing!”   A great way to cut calories!

How come a private company is funding national nutrition surveys in Asia?

I was surprised to read a report in FoodNavigator.com that a private company is about to conduct an enormous—and undoubtedly very expensive—study of the nutritional status of children in Southeast Asia.

The study will collect data from more than 16,000 children aged 12 and under in four countries:

  • Dietary profiles and nutrient intake assessment, including food intake, bone density and cognition.
  • Iron status, vitamins, lipid profile and blood pressure.
  • Body composition and physical activity, including measurements on weight, height and hand grip strength.

The company is doing this in partnership with institutions in Indonesia, Malaysia, Thailand, and Vietnam.

Why would a private company embark on a project like this?  The company is FrieslandCampina, a Dutch firm specializing in dairy products:

We provide people around the world with all the good things milk has to offer, with products that play an important role in people’s nutrition and well-being.

Our product range: baby and infant food, milk-based drinks, cheese, milk, yoghurts, desserts, butter, cream, milk powder, dairy-based ingredients and fruit-based drinks.

As the company explains, “We aspire to help people move forward in life with our dairy nutrition, and are committed to helping our consumers maintain and improve their nutritional well-being with the goodness of milk.”

I’m willing to predict that these studies will show that kids in Southeast Asia would be a lot healthier if they drank more milk.   And will find reasons to dismiss concerns that lactose intolerance is the norm in Asian populations over the age of five or so.

Don’t farmworkers’ kids deserve a break?

I’ve been mulling over the article in the New York Times (March 13) about the effects of an itinerant lifestyle and the threat of deportation on the children of farmworkers in California.  If ever there was an example of how the political gets personal, this is it.

The article focuses on a third-grade teacher, Oscar Ramos, who is on the front lines trying to give these kids a chance in life, let alone at the American dream.  It describes what he’s up against: nearly all his students are near the poverty line, and nearly 80% have limited English.  They move frequently and live under crowded conditions.

But the often disrupted lives of the children of migrants here is likely to grow still more complicated as the national debate over immigration grows sharper.

Efforts by lawmakers to rescind automatic citizenship for children born in the United States to illegal immigrants are already stoking fears among many agricultural workers, and that has consequences for their children.

Some parents, as they move with the crops, are already keeping their children out of school when they get to Arizona because they are worried about the bureaucracy and tougher restrictions in the state.

The article is long but well worth reading.  If nothing else, take a look at the photographs.

This is how our relatively inexpensive food gets to us.   The costs, as the economists tell us, are externalized.  Here is one of those externalized costs–the potential of those kids to become functioning citizens in our democratic society.
