Five more sponsored studies with expected results. The score: 80:6

In case you haven’t been following this saga, I’ve been collecting studies funded by food companies since last March and posting them according to whether the results do or do not favor the sponsor’s interests.  So far, I’ve reported 80 that do, versus 6 that don’t (although I have a couple more saved up for posting soon).

The point of this exercise is to demonstrate that sponsored studies are far more likely to favor the interests of the sponsor than are studies funded by government agencies or foundations, and to argue for more independent funding of food and nutrition research (see my recent Viewpoint on this topic in JAMA Internal Medicine).

Here are five more examples.

Is consuming yoghurt associated with weight management outcomes? Results from a systematic review. J Eales, I Lenoir-Wijnkoop, S King, H Wood, F J Kok, R Shamir, A Prentice, M Edwards, J Glanville, R L Atkinson. Int’l J Obesity 2015 ahead of print.

  • Conclusions: Yoghurt consumption is associated with lower BMI, lower body weight/weight gain, smaller waist circumference and lower body fat in epidemiological studies. RCTs [randomized controlled trials] suggest weight reduction effects, but do not permit determination of a cause/effect relationship. Well-controlled, adequately powered trials in research and community settings appear likely to identify a modest but beneficial effect of yoghurt consumption for prevention of weight gain and management of obesity. The ready availability of yoghurt (a nutrient dense food) and its ease of introduction to most diets, suggests that educating the public to eat yoghurt as part of a balanced and healthy diet may potentially contribute to improved public health.

  • Conclusion:  The state of the epidemiologic science on red meat consumption and CRC [colorectal cancer] is best described in terms of weak associations, heterogeneity, an inability to disentangle effects from other dietary and lifestyle factors, lack of a clear dose-response effect, and weakening evidence over time.
  • Funding: This manuscript was partially funded by Meat & Livestock Australia (MLA). MLA did not contribute to the writing, analysis, or interpretation of study findings.
  • Comment: The conclusion of this industry-funded study contradicts the findings of the WHO International Agency for Research on Cancer, which finds processed meats to be carcinogenic, and red meat to be a probable carcinogen.

  • Thrust of paper: As every DGAC [Dietary Guidelines Advisory Committee] wants to be bold and set new direction, nutrition science would support that first we must do no harm with our dietary guidance. Moderation and variety must be kept front and center…A suggestion that all Americans should reduce sodium intakes is not sound and is potentially dangerous. Targeting certain foods and beverages, including chocolate milk, processed meats, added sugars, and even the noble potato as villains in the nutrition wars is not a science-based strategy and may need to be countered on the political front if appointed scientific review committees continue to take this approach.

  • Conclusions: Overall, the balance of evidence indicates that use of LES [low-energy sweeteners] in place of sugar, in children and adults, leads to reduced EI [energy intake] and BW [body weight], and possibly also when compared with water.
  • Conflict of interest: Peter J Rogers has received grants from Sugar Nutrition, UK in support of research on the effects of sugar on human appetite. Cees de Graaf has received grants from the Dutch Sugar Bureau in support of a study on brain responses to sugars and low energy sweeteners. Walnut ingestion in adults at risk for diabetes: effects on body composition, diet quality, and cardiac risk measures Valentine Yanchou Njike, Rockiy Ayettey, Paul Petraro, Judith A Treu, David L Katz.  . BMJ Open Diabetes Research and Care 2015;3:e000115. doi:10.1136/bmjdrc-2015- 000115.

  • Conclusions: The inclusion of walnuts in an ad libitum diet for 6 months, with or without dietary counseling to adjust calorie intake, significantly improved diet quality, endothelial function, total and LDL cholesterol, but had no effects on anthropometric measures, blood glucose level, and blood pressure.
  • Funding: Funding for this study has been provided by the California Walnut Commission.
  • Competing interests. DLK has been compensated for public speaking by the California Walnut Commission.

Does eating eggs raise blood cholesterol levels?

The Physicans Committee for Responsible Medicine (PCRM), a group advocating against use of animals in research but for vegetarian and vegan diets, has started a campaign to restore egg-and-cholesterol recommendations to the Dietary Guidelines for Americans.

Eggs are the largest source of cholesterol in American diets.

The campaign involves billboards like this one, in six locations in Texas:

It also involves a new organization (truthaboutegg.org) with an interactive website on a dozen issues related to egg production and consumption.

The one that particularly caught my eye was #5.

A 2013 review suggested that high-cholesterol foods have only a modest effect on blood cholesterol. Of the 12 studies it relied on, 11 were industry-funded.

In a letter to Congressman K. Michael Conaway (Rep-TX), Dr. Neal Barnard, PCRM’s president, wrote:

This week, billboards near your Texas offices will alert you to the dangers Americans face if cholesterol warnings are removed…Eggs are the leading source of cholesterol in the American diet.  A report (which I’ve included for your review) in the autumn 2015 Good Medicine magazine finds that this recommendation may have been influenced by egg-industry-funded cholesterol research. America’s heart disease and diabetes epidemics will continue unabated if the egg industry succeeds in its efforts to get cholesterol warnings out of the guidelines.

The 2015 Dietary Guidelines Advisory Committee (DGAC) said this about dietary cholesterol.

Cholesterol. Previously, the Dietary Guidelines for Americans recommended that cholesterol intake be limited to no more than 300 mg/day. The 2015 DGAC will not bring forward this recommendation because available evidence shows no appreciable relationship between consumption of dietary cholesterol and serum cholesterol, consistent with the conclusions of the AHA/ACC report.2,35 Cholesterol is not a nutrient of concern for overconsumption.

The DGAC based its unconcern about dietary cholesterol on two references:

35.  Shin JY, Xun P, Nakamura Y, He K. Egg consumption in relation to risk of cardiovascular disease and diabetes: a systematic review and meta-analysis. Am J Clin Nutr. 2013;98(1):146-59. PMID: 23676423. This study, which was also independently funded, concluded:

compared with those who never consume eggs, those who eat 1 egg per day or more are 42% more likely to develop type 2 diabetes. Among diabetic patients, frequent egg consumers (ie, > 1 egg/d) are 69% more likely to have CVD comorbidity…This meta-analysis suggests that egg consumption is not associated with the risk of CVD and cardiac mortality in the general population. However, egg consumption may be associated with an increased incidence of type 2 diabetes among the general population and CVD comorbidity among diabetic patients.

Were these references based largely on studies funded by the egg industry?  If so, PCRM is correct in arguing that the question of egg consumption and blood cholesterol levels merits much closer scrutiny and analysis than it is currently receiving.

What does a study funded by the egg industry look like?  Here are two one from my recent collection:

The effect of a high-egg diet on cardiovascular risk factors in people with type 2 diabetes: the Diabetes and Egg (DIABEGG) study—a 3-mo randomized controlled trial, by Nicholas R Fuller, Ian D Caterson, Amanda Sainsbury, Gareth Denyer, Mackenzie Fong, James Gerofi, Katherine Baqleh, Kathryn H Williams, Namson S Lau, and Tania P Markovic.  Am J Clin Nutr 2015; 101:705-713.

  • Conclusion: High egg consumption did not have an adverse effect on the lipid profile of people with T2D [type 2 diabetes] in the context of increased MUFA [monounsaturated fatty acid] and PUFA [polyunsaturated fatty acid] consumption. This study suggests that a high-egg diet can be included safely as part of the dietary management of T2D, and it may provide greater satiety.

  • Conclusion: Reviewed studies were heterogeneous and lacked the methodologic rigor to draw any conclusions regarding the effects of dietary cholesterol on CVD risk.  [Implication: suggestions that eggs might raise cardiovascular risk are unwarranted]
  • Sponsor: Supported by USDA agreement 1950-51000-073 and the American Egg Board, Egg Nutrition Center.  The funders did not have a role in the study selection, quality assessment, data synthesis, or manuscript preparation.

Some comments on the meat-is-carcinogenic report

Yesterday, the World Health Organization’s International Agency for Research on Cancer (IARC) issued a warning about the carcinogenic potential of processed and red meat.  This, as you might expect, caused a media flurry.  CNN News asked me for a written comment.  They titled it “The other benefit to eating less red meat.”  Here’s what I wrote:

The just-released report from the International Agency for Research on Cancer judging processed meat as clearly carcinogenic and red meat as probably carcinogenic has caused consternation among meat producers and consumers.

Meat producers do not like the “eat less meat” message. Consumers do not want to give up their bacon and hamburgers — delicious and also icons of the American way of life.

But these judgments should come as no surprise to anyone. Eating less processed and red meat has been accepted dietary advice since Ancel and Margaret Keys wrote their diet book for heart disease prevention, “Eat Well and Stay Well,” in 1959. Their advice: “restrict saturated fats, the fats in beef, pork, lamb, sausages …” They aimed this advice at reducing saturated fat to prevent heart disease. Federal committees and agencies have continued issuing such heart-disease advice to the present day.

Cancer entered the picture in the 1970s, when scientists began to link red meat — beef, pork, lamb — to the risk of cancers of the colon and rectum. Even after several decades of research, they had a hard time deciding whether the culprit in meat was fat, saturated fat, protein, carcinogens induced when meat is cooked to high temperatures or some other component.

In the mid-1990s, dietary guidelines committees advised eating lean meats and limiting intake of processed meats, still because of their high fat content. By the late 1990s, cancer experts said that red meat “probably” increases the risk of colorectal cancers, and “possibly” increases the risk of cancers of the pancreas, breast, prostate and kidney. The IARC report, based on more recent evidence, makes even stronger recommendations and favors carcinogens as the causative factors.

To put this in context: For decades, the meat industry’s big public relations problem has been that vegetarians are demonstrably healthier than meat eaters. People who do not eat red meat havemuch less of a chance of developing heart disease and bowel cancers than the average American.

More recently, the 2015 Dietary Guidelines Advisory Committee (DGAC) found diets “higher in red/processed meats…” to be associated with a greater risk of colorectal cancer, and it recommended dietary patterns and low in red and/or processed meats, but higher in vegetables, fruits, legumes, whole grains, lean meats/seafood and low-fat dairy — largely, but not necessarily exclusively, plant-based.

This is good advice for anyone.

Eating less red and processed meats has two benefits: a reduced risk for certain forms of cancer,and a reduced effect on climate change.

The DGAC deemed eating less red meat to be exceptionally beneficial to the environment as well as to human health. The IARC report strengthens the health component of the recommendation. The secretaries of USDA and Health and Human Services, however, have refused to allow environmental concerns to be considered in the 2015 dietary guidelines.

I mention the dispute over environmental “sustainability” in the dietary guidelines because largely plant-based diets are appropriate for all kinds of health concerns — obesity, type 2 diabetes, coronary heart disease and now, especially, colorectal cancer — as well as environmental concerns.

By eating less red and processed meats, you promote both your own health and that of the planet.

At issue then is how much red and processed meat is compatible with good health. The IARC commission ducked that question, although it cites evidence that as little as 100 grams (a quarter pound) of red meat a day, and half that much of processed meats, increases cancer risk by 15% to 20%.

Will an occasional hamburger or piece of bacon raise your risk that much? I don’t think so. But the evidence reviewed by IARC strongly suggests that if you do eat meat, eat less when you do, don’t eat meat every day, save processed meats for rare treats and be sure to eat plenty of vegetables.

Fortunately, this advice leaves plenty of room for delicious meals — just with meat taking up much less room on the plate.

100 Mayors Sign Milan Urban Food Policy Pact

This morning, I received this press release from Franca Roiatti in Milan, announcing that on October 15 the mayors of more than 100 cities signed the Milan Urban Food Policy Pact and Framework for Action.  This pact commits these cities—New York among them—to work for more equitable and sustainable urban food systems.

The mayors made 7 commitments, among them working to

  • Develop sustainable food systems that are inclusive, resilient, safe and diverse, that provide healthy and affordable food to all people in a human rights-based framework, that minimise waste and conserve biodiversity while adapting to and mitigating impacts of climate change;
  • Engage all sectors within the food system (including neighbouring authorities, technical and academic organizations, civil society, small scale producers, and the private sector) in the formulation, implementation and assessment of all food-related policies, programmes and initiatives;
  • Use the Framework for Action as a starting point for each city to address the development of their own urban food system and we will share developments with participating cities and our national governments and international agencies when appropriate;

The Framework recommends 37 actions, among them

  • Identify, map and evaluate local initiatives
  • Develop or revise urban food policies and plans
  • Address non-communicable diseases associated with poor diets and obesity, giving specific attention where appropriate to reducing intake of sugar, salt, transfats, meat and dairy products and increasing consumption of fruits and vegetables and non-processed foods
  • Develop sustainable dietary guidelines to inform consumers, city planners (in particular for public food procurement), food service providers, retailers, producers and processors, and promote communication and training campaigns.
  • Explore regulatory and voluntary instruments to promote sustainable diets involving private and public companies as appropriate, using marketing, publicity and labelling policies; and economic incentives or disincentives; streamline regulations regarding the marketing of food and non-alcoholic beverages to children in accordance with WHO recommendations.
  • Those aimed at social and economic equity (cash transfers, school feeding programs, employment, education, training, research).
  • And those aimed at improving food production and reducing waste.

Finally, it comes with an e-book that collects 49 selected good practices from 28 signatory cities.

The point?  Even though everything about this pact and framework is voluntary, these findings and recommendations ought to be enough to give any city mayor a mandate to start working on sustainability issues.

I am looking forward to seeing how New York City uses the report and framework.

Sugars for toddlers: an invitational roundtable from The Sugar Association

This week, I’m going to be posting items about sugar politics.

Item on sugars #1:

Funny thing.  I was not invited to this event, but someone who was invited passed along the invitation.  You too will be sad you weren’t invited.

I am contacting you at the request of Dr. Courtney Gaine, VP of Scientific Affairs from The Sugar Association, regarding an invitational roundtable on The Role of Sugars in Supporting a Nutrient-dense Diet for Toddlers, 12 to 24 Months.  It will be sponsored by the University of Colorado, Anschutz Medical Campus, Department of Pediatrics, chaired by Dr. Ronald Kleinman from Harvard Medical School, co-chaired by Dr. Frank Greer from the University of Wisconsin School of Medicine, and facilitated by Sylvia Rowe.  The roundtable is supported by an unrestricted educational grant from the Association….

Roundtable Objectives

  • Provide a forum to discuss the science and research voids on the role of sugars as a strategy that may help parents successfully transition their older infants and toddlers (12 to 24 months) from complementary infant foods to consuming a variety of nutrient-dense foods from the family table.
  • Generate potential research ideas and questions on this topic for future guidance on the feeding of young children, including birth to 24 months, which is scheduled for integration into the 2020 Dietary Guidelines for Americans.
  • Create the impetus to extend this research to public-private partnerships with industry, academy and the government.

Proposed Topic Areas 

  • The roundtable has been tentatively divided into these 5 topic areas: 1) transitional toddler feeding and nutrition policy; 2) physiology; 3) sugars in toddler feeding practices; 4) parent-feeding strategies: emerging science; and 5) the research path forward….

Honorarium 

The Sugar Association will reimburse you for all reasonable travel expenses, plus a $2,000 honorarium for your review of abstracts and presentations, which you will receive in mid-October, and your participation in the 1 ½ day roundtable.

This requires some translation.  I may be over-interpreting here, but as I see it, the Sugar Association is paying academics $2000 to implicitly endorse:

  • Promoting the use of sugar as a way to get toddlers to eat healthier foods.
  • Making sure the 2020 dietary guidelines say nothing about the need for kids to eat less sugar (we don’t even have the guidelines for 2015 yet).
  • Making sure that government agencies don’t advise or set policies to encourage eating less sugar.

Sigh.

Five more industry-funded studies with expected results. Score: 47 to 1

The Inadmissibility of What We Eat in America and NHANES Dietary Data in Nutrition and Obesity Research and the Scientific Formulation of National Dietary Guidelines. Archer EPavela GLavie CJ. Mayo Clin Proc. 2015 Jul;90(7):911-26. doi: 10.1016/j.mayocp.2015.04.009. Epub 2015 Jun 9.

  • Conclusion: we conclude that M-BM [memory-based dietary assessment methods] data cannot be used to inform national dietary guidelines and that the continued funding of M-BMs constitutes an unscientific and major misuse of research resources.
  • Funding: National Institute of Diabetes and Digestive and Kidney Diseases of the National Institutes of Health.
  • Potential Competing Interests: Dr Archer has received honoraria from the International Life Sciences Institute and The Coca Cola Company. Dr Lavie reports receiving consulting fees and speaking fees from The Coca-Cola Company….
  • Comment: This is part of what appears to be a concerted effort by Coca-Cola to discredit NHANES, the national survey of dietary intake and disease risk that consistently associates soda intake to poor health.

  • Conclusions: The daily addition of a HP [high-protein] breakfast improved indices of weight management as illustrated by the prevention of body fat gain, voluntary reductions in daily intake, and reductions in daily hunger in breakfast skipping adolescents with overweight/obesity.
  • Funding: The Pork Checkoff supplied the funds to complete the study.
  • Disclosure: The authors declared no conflict of interest.
  • Comment: Industry-funded investigators typically state that funding does not introduce conflicts of interest.

Breakfasts Higher in Protein Increase Postprandial Energy Expenditure, Increase Fat Oxidation, and Reduce Hunger in Overweight Children from 8 to 12 Years of Age.  Jamie I Baum, Michelle Gray, and Ashley Binns.  J.   • Conclusion: This study indicates that breakfast macronutrient composition affects postprandial responses in both NW [normal weight] and OW [overweight] children. A PRO [protein-rich breakfast] increases postprandial EE [energy expendititure] and fat oxidation, reduces hunger, and increases satiety when compared with a carbohydrate-based breakfast.
  • Funding: Supported by a grant from the Egg Nutrition Center/American Egg Board, Chicago, IL. The Egg Nutrition Center/American Egg Board was not involved in the design, implementation, analysis, or interpretation of the data.
  • Author disclosures: JI Baum, M Gray, and A Binns, no conflicts of interest.
  • Comment: These industry-funded investigators also deny that funding introduces conflicts of interest.

The Effect of Breakfast vs. No Breakfast on Brain Activity in Adolescents when Performing Cognitive Tasks, as Assessed by fMRI [functional magnetic resonance imaging].  Jonathan Fulford1, Joanna L Varley2 and Craig A Williams.   Nutritional Neuroscience 2015 epub ahead of print.

  • Conclusion: Although no statistically significant (P > 0.05) improvement in task performance was determined, significantly higher activation was recorded in the frontal, premotor, and primary visual cortex areas in the breakfast trial relative to the fasting condition…Such a finding may have important implications in the examination of the role of diet, and specifically breakfast, in determining children’s performance within the school environment.
  • Funding: A grant was received of £18,678.08 from Kellogg Marketing & Sales Company (UK) Ltd to cover MRI scanning costs. Otherwise the research was conducted with the support of internal institutional funds and the authors received no other direct or indirect support, with no further competing interests.
  • Comment: Ordinarily I’m not concerned about food companies’ donating products to be tested but £18,678.08 seems noteworthy, especially since the only point of this study is to demonstrate that breakfast-eaters do better (higher brain activation even though no significant gain in task performance).

Suboptimal Serum α-Tocopherol Concentrations Observed among Younger Adults and Those Depending Exclusively upon Food Sources, NHANES 2003-2006. McBurney MI, Yu EA, Ciappio ED, Bird JK, Eggersdorfer M, Mehta S (2015). PLoS ONE 10(8): e0135510. doi:10.1371/journal.pone.0135510

  • Conclusion: The prevalence of inadequate vitamin E levels is significantly higher among non-users of dietary supplements…Our findings provide evidence that most Americans have serum α-tocopherol levels below 30 μmol/L. The EAR [Estimated Average Requirement], epidemiological and randomized controlled studies all indicate that maintaining a serum α-tocopherol concentration of 30 μmol/L may have beneficial effects on mortality, cognitive function and reproduction [Note: “may” indicates that what follows is speculative].
  • Funding: This statistical analysis of data collected by the Centers for Disease Control (CDC)…was supported by DSM Nutritional Products, a manufacturer of vitamin E. MM, EC, JB and ME are employees of DSM Nutritional Products. DSM Nutritional Products provided support in the form of salaries for authors MM, EC, JB, and ME and as an unencumbered gift to Cornell University that was used to support EY as a graduate research assistant.
  • Competing interests: This study was supported by DSM Nutritional Products, a manufacturer of vitamins, including vitamin E, for food, dietary supplement, and pharmaceutical use. MM, EC, JB, and ME are employees of DSM Nutritional Products.
  • Comment: Inadequate vitamin E in this study is defined as a serum level below a certain cut-point with uncertain clinical significance.  According to the Dietary Reference Intakes, clinical signs of vitamin E deficiency have not been observed in healthy populations.

Note: Since mid-March, I have posted 47 industry-funded studies with results favorable to food companies or trade associations, vs. 1 study with unfavorable results.

Update on the school meal situation

School is starting and the school food debates will no doubt be starting up again.

The USDA has a new report on what’s happening with adoption of the new nutrition standards.

  • A national study of elementary school principals and foodservice managers finds the majority (63%) to agree or strongly agree (7%) that students seem to like the new lunches.
  • The participation rate for paid school lunches declined from FY 2008 through FY 2014, with steeper declines during FY 2012-2014.  This could be do to the changes in standards but is more likely the result of higher prices charged for meals.
  • Smaller, more rural, and wealthier districts had the most difficulty adopting the new meal standards. Higher meal prices affected smaller and more rural districts.

The 2015 School Food Poll conducted by the Kellogg Foundation just reported:

  • 90% of respondents support the national school nutrition standards.
  • 86% say the school nutrition standards should stay the same or be strengthened.
  • 91% say kids need access to safe drinking water in schools.
  • 88% support increased government funding to expand farm to school programs.
  • 84% believe sustainable agriculture should be part of the Dietary Guidelines for Americans.

My interpretation: The nutrition standards are working well enough but it’s time to advocate even more strongly for universal school meals.  It’s absurd and unconscionable that poor kids are getting priced out of school meals.

Addition: The School Nutrition Association, the group doing everything it can to undermine the new nutrition standards, has just issued a report finding that the standards have caused severe financial harm to 70% of schools.  The reason?

There is strong consensus as to the leading reason for the decline in lunch ADP: decreased student acceptance of meals [.underlined in report].

The report does, however, provide a table of reasons for increased costs:

  • Increased per meal food costs — 70.1%
  • Decreased lunch participation — 56.8%
  • Decreased a la carte revenue — 53.0%
  • Increased labor/benefits costs — 48.5%
  • Declining student enrollment — 20.6%

Given these results, you might think the SNA would be lobbying night and day for higher reimbursement rates, but no such luck.  The SNA is lobbying for weaker standards.  Pity.

Addition, August 25:  A study from the University of Vermont finds school kids to be consuming slightly fewer servings of fruits and vegetables since the nutrition standards were implemented and to be producing 56% more plate waste.  This is not good news.

Addition, August 28:  But a CDC study finds that in 2014, schools were making significant progress:

  • Almost all schools offered whole grain foods each day for breakfast (97.2%) and lunch (94.4%)
  • Most schools offered two or more vegetables (79.4%) and two or more fruits (78.0%) each day for lunch.
  • Approximately one third (30.5%) of schools offered self-serve salad bars.
  • Among the 55.0% of schools that prepared food at the school, about half were trying hard to reduce salt.
  • Overall, 97.5% of schools used at least one of the nine school nutrition services practices examined, with 23.9% using one to three of the practices, 48.3% using four to six of the practices, and 25.3% using seven or more of the practices.

My interpretation: Schools are moving to adopt the new nutrition standards.  Some are succeeding better than others.  The outcome of studies therefore depends on whether you see the glass as half full, or empty.

More industry-sponsored research with predictable results

  • Conclusion:  …although it appears prudent to avoid excessive consumption of fructose-containing sugars, levels within the normal range of human consumption are not uniquely related to CVD risk factors with the exception of triglycerides, which may rise when simple sugars exceed 20% of energy per day, particularly in hypercaloric settings.  [My translation: this implies it’s OK to eat sugars up to 20% of calories per day, even though health authorities typically recommend 10% or less].
