The FTC’s latest report on food marketing to kids: glass half full or empty?

Last Friday was a big day for releasing reports that federal agencies would rather keep quiet.  As I discussed in my previous post, the FDA released its long overdue environmental impact report on GM Salmon.

Today, let’s take a look at the FTC’s latest report on the state of food and beverage marketing aimed at children. This is a follow-up to the report the FTC issued in 2008.

The new report cites progress.  From 2006 to 2009, its press release says:

  • Total spending on food marketing to youth ages 2-17 dropped from $2.1 billion to $1.79 billion, mainly because of less spending on television advertising.
  • But spending on new media, such as online, mobile, and viral marketing, increased by 50 percent.
  • Cross-promotions that link marketed foods with popular children’s movies and TV characters increased from 80 children’s movies and TV shows to 120.
  • Cereals had 0.9 gram less sugar and 1.6 grams more whole grain.
  • Fast food was a  little lower in calories, sodium, sugar, and saturated fat.

The report concludes:

Despite the commendable progress, this report identifies areas where further efforts could be made by the food and beverage industry and the media industry to improve the nutritional quality of foods marketed to youth.

Whether you see this as good news or not such good news depends on whether you see the glass as half full or half empty.

If you are a half-full type, you will rejoice that food companies are voluntarily improving the nutritional quality of their products even if the improvements are small.

If you are a half-empty type (which, in this case, I am), you will be dismayed at the lack of real progress in reducing the marketing of junk foods to kids.

Companies must sell more and more products to grow their businesses.  They are under pressure to reduce such marketing and to improve the quality of the products they sell.  Neither change is likely to increase sales.   Hence: resistance to change.

As Andy Bellatti points out, small nutritional improvements are unlikely to have much of an effect on health: reducing the sugars by 0.9 grams can be interpreted as “better for you” but does not necessarily make a product a good choice.

And as Michele Simon’s interview with former marketer Bruce Bradley suggests, the data on which the FTC based its positive assessments may well have been gamed by the companies reporting it.

As I read this report, it provides plenty of evidence that stopping food and beverage marketing to kids is the issue that matters most to doing something to reverse childhood obesity.

The election is over.    Maybe Let’s Move! can revisit the marketing-to-kids issue.  Someone needs to do it.  And soon.

Low carb or low fat: Do calories count?

Here’s my once a month on the first Sunday Food Matters column for the San Francisco Chronicle, out today:

Q: I’m confused about calories. If I cut calories to lose weight, does it matter what foods I eat? Or are all calories the same?

A: As the co-author of “Why Calories Count: From Science to Politics,” I hear this question all the time.

The short answer: Calories matter for weight. The choice of foods that provide the calories matters a lot for health and may make it easier for you to diet successfully.

To lose weight, reducing calorie intake below expenditure works every time.

To prove this point, a professor at Kansas State University lost 27 pounds in 10 weeks on the Twinkies diet – one Twinkies every three hours with occasional snacks of chips, sugary cereals and cookies. Even so, he cut his usual calorie intake by 800 a day. Anyone would lose weight doing that.

Only four dietary components provide calories: fat (9 per gram), carbohydrate and protein (4 per gram each) – and alcohol (7 per gram).

Does the particular mix of these components make any difference to weight loss? Yes, say proponents of diets low in carbohydrate, especially rapidly absorbable sugars and refined starches.

Low-carbohydrate diets are necessarily high in fat, and somewhat higher in protein. Do people lose weight on them because of the effects of carbohydrates on insulin levels or because low-carbohydrate diets help reduce calories?

This question does not have an easy answer, but not for lack of trying. Weight-loss studies are hard to do. Estimating calorie intake is notoriously inaccurate, and measuring calories is difficult and expensive.

The first measurement study I know of took place in 1964. Investigators from the Oakland Institute for Medical Research studied weight loss in five obese patients in a hospital metabolic ward. They calculated the number of calories needed to induce rapid weight loss in each patient, and fed each of them a liquid formula diet containing that number every day. Every few weeks, they changed the formula to vary the proportions of protein (ranging from 14 percent to 36 percent of calories), fat (12 percent to 83 percent), and carbohydrate (3 percent to 64 percent).

Regardless of the proportions, all patients lost weight at a constant rate. The investigators titled their study “Calories Do Count.”

This study was conducted under rigidly controlled conditions of hospitalization and involved actual measurements – not estimations – of calorie intake and body weight.

But what about weight-loss studies involving people who are not incarcerated? Since the early 2000s, numerous clinical trials have shown low-carbohydrate diets to produce greater weight loss than low-fat diets. Some also have observed improvements in blood pressure, blood glucose levels and blood lipids.

But it is so inaccurate to estimate calorie intake in such studies that most didn’t bother to try. This means they can’t say whether the weight loss was due to composition of the diet or to calorie reduction.

It’s possible that low-carbohydrate, high-fat diets make people less hungry, but the evidence for this is mixed. Most studies of extreme diets of any type report high dropout rates or failure to stick to the diet for more than six months or so. And even though initial weight loss is rapid on low-carbohydrate diets because of water loss, these diets are low in fiber and some vitamins.

One problem with losing weight is that it takes fewer calories to maintain smaller bodies. Dieting also reduces energy expenditure.

One recent study of that problem involved taking detailed measurements for several years, and illustrates the difficulties of obtaining definitive answers to questions about diet composition and energy balance.

The researchers wanted to know whether diet composition affected energy expenditure in very obese people who had just dieted off up to 15 percent of their weight. They found that a low-carbohydrate diet did not slow down energy expenditure nearly as much as a low-fat diet, meaning that low-carbohydrate diets might make it easier for people to maintain weight loss.

On this basis, the investigators said, “The results of our study challenge the notion that a calorie is a calorie from a metabolic perspective.”

Perhaps, but study subjects were fed prepared calorie-controlled diets for only four weeks, and lost and maintained weight under highly controlled conditions. Does diet composition matter for weight maintenance in the real world? Longer-term studies by other investigators show that diet composition makes little difference in the ability to maintain weight loss.

Most reviews of the subject conclude that any diet will lead to weight loss if it cuts calories sufficiently.

Obviously, some diets are better for health than others.

Face it. The greatest challenge in dieting is to figure out how to eat less – and to eat healthfully on a regular basis – in the midst of today’s “eat more” food environment. And that’s a much more important research problem than whether low-carbohydrate or low-fat diets work better for weight loss.

Summer reading: reports on diet and health

It’s the (relatively) quiet season and I’m getting caught up on reports coming in.   Here are two.

1.  The Bipartisan Policy Center, a group founded by former cabinet secretaries, has come up with a plan to improve the health of Americans: Lots to Lose: How America’s Health and Obesity Crisis Threatens our Economic Future.   The Executive Summary is online, but the website is difficult to navigate and you have to log into Facebook to read the entire report.

The report calls on the public and private sectors to collaborate in creating healthy families, schools, workplaces and communities. Some of the recommendations are aimed at the food environment, rather than individuals, which is good.  And they are addressed to families, schools, workplaces, communities, and farm policy.  But like most such reports this one does not explain how any of its recommendations might be achieved.

2.  The Rudd Center at Yale has produced Cereal Facts, a study showing that cereal companies:

Increased media spending on child-targeted cereals by 34% from 2008 to 2011, mainly on the least nutritious products.

  • More than doubled spending in Spanish-language media.
  • Improved overall nutritional quality of 13 of 14 brands advertised to children by 10 percent on average.
  • Sponsor TV ads that typically promote products containing one spoonful of sugar for every three spoonfuls of cereal.

Two more findings of interest:

  • In 2011, the average 6- to 11-year-old saw more than 700 TV ads for cereals.
  • Although General Mills and Kellogg do make nutritious products that are marketed to parents, they do not advertise those products to children.

Three terrific books about feeding kids

Karen Le Billon, French Kids Eat Everything: How our family moved to France, cured picky eating, banned snacking, and discovered 10 simple rules for raising happy, healthy eaters, HarperCollins, 2012.

I blurbed this one: It takes a brave couple to move two picky-eater kids to a small French town and convert them to foodie omnivores.  North Americans have much to learn from European food traditions, and the contrast between French and North American school lunches is a striking example.  A must-read for teachers as well as parents.

Jeannie Marshall, Outside the Box: Why Our Children Need Real Food, Not Food Products, Random House, 2012.

I blurbed this one too: Outside the box is about teaching kids how to appreciate real food but also about how globalization is changing the way the world eats.  In this beautifully writeen book about what needs to be done to preserve food culture in Italy and elsewhere, Marshall makes the political personal as she explains how she is teaching her son to enjoy the pleasures of eating food prepared, cooked, and lovingly shared by friends and family.

Bill and Claire Wurtzel, Funny Food: 365 Fun, Healthy, Silly, Creative Breakfasts, Welcome Books, 2012.

I wasn’t asked to blurb this one, but like it anyway:  Your kid doesn’t want to eat real food for breakfast?  No excuses.  This book illustrates 365 breakfasts made of eggs, toast, cereal, pancakes, fruit, cheese, yogurt, and other good things.  These are presented as faces, animals, and toys so easy to do that even a time-challenged parent can whip them up in a second.  I can’t imagine any kid resisting eating foods like these.    Silly, absolutely.  Worth it?  Give it a try.

Nutritionist’s Notebook: Importance of fiber

This semester I answer students questions about nutrition on Tuesdays in the student-run Washington Square News.  Today’s is about fiber.

Question: We tweeted and linked to the article you were cited in The New York Times on Tuesday, and you mentioned the importance of fiber in a diet. How much fiber is good to have in a daily diet, and what does it do for our bodies? What are good sources of fiber — cereal bars, granola bars? How much is too much fiber?

Answer: You, like most Americans, probably get less than half the fiber you need. You can fix that by eating more vegetables, fruits, nuts, beans, peas and whole grain breads and cereals. Fiber occurs only in plants.

Eating these foods at every meal will do wonders for your digestive system. They help protect you against obesity, heart disease, Type 2 diabetes and even some cancers.

Fiber refers to a bunch of plant carbohydrates that human enzymes cannot digest easily. These come in two types: insoluble and soluble. Insoluble fiber, the kind found in grains, almost completely resists digestion. It has no calories.

But soluble fiber from beans and other vegetables can be digested to some extent by bacteria in your colon. They produce a little energy you can use.

Research shows that people who eat plenty of vegetables, fruits and whole grains with both kinds of fiber develop less chronic disease. But these benefits do not show up in studies using fiber supplements.

This means you are better off eating vegetables than power bars. If you must eat those bars, choose the ones with fruits, nuts and whole grain — real foods — high up on the ingredient list. Watch out for misleading claims about “good source of fiber.”  Check the Nutrition Facts panels — 3 grams is minimal, 5 is better.

If you are not used to eating fruits, vegetable and grains, eating more of them may make you feel gassy. Gradually include more. Your digestive system will be happier, and you will be healthier.

Walmart’s new front-of-package “buy me” logo

This morning, Walmart announced a new FOP labeling program:

The logo will go on Walmart’s in-house brand products that meet the company’s nutritional criteria.  These criteria are similar (but not identical) to those recommended by the Institute of Medicine (IOM) in its recent report advising the FDA about what should be included in front-of-package labels.

Because the FDA has not yet acted on the IOM report, Walmart—like other retailers—is jumping the gun in doing its own thing.  Its thing, however, is a substantial improvement over the Facts Up Front scheme put in place by the Grocery Manufacturers Association and Food Marketing Institute.

In general, strict nutrition criteria for salt, sugar, and saturated fat exclude most supermarket products.

Walmart’s criteria are pretty strict.  They exclude 80% of Great Value products.

In the cereal category, for example, only these Great Value items qualify:

  • Extra Raisin Bran Cereal
  • Raisin Bran
  • Bran Flakes
  • Crunchy Oat Squares
  • Frosted Shredded Wheat
  • Crunchy Nugget Cereal
  • Toasted Wholegrain Oat Cereal

But these Great Value cereals do not:

  • Cocoa Cool Cereal
  • Cinnamon Crunchy Oat Squares Cereal
  • Apple Blasts Cereal
  • Sugar Frosted Flakes Cereal
  • Toasted Corn Cereal
  • Crisp Rice Cereal
  • Fruit Spins Cereal
  • Fruity Puffs Cereal
  • Crunchy Honey Oats Cereal
  • Vanilla Almond Awake Cereal

OK, but I wish the company had waited for the FDA to decide on a plan for FOP labeling (and I wish the FDA would get busy on that plan).

All of these schemes are ways to avoid putting negative information on package labels.  No seller or retailer wants a red traffic light—“don’t buy me”—on its products, especially because research shows that stop signals work.  Customers tend not to buy products marked with red traffic lights.

The IOM report concluded that negatives (“don’t buy”) worked better than positives (“buy me”) in guiding consumer choice.   A more recent study confirms that finding.

Companies much prefer green-light systems like the one Walmart is doing.

The Walmart press release explains:

Walmart moms are telling us they want to make healthier choices for their families, but need help deciphering all the claims and information already displayed on products…Our ‘Great For You’ icon provides customers with an easy way to quickly identify healthier food choices…this simple tool encourages families to have a healthier diet.

But does it?  Will Walmart customers buy more of the items marked with the logo instead of the other kinds?  The company says it is doing the research.  Will customers who buy products with the logo be healthier as a result?

I can’t wait to find out.

Consumer Reports’ arsenic-in-juice study: what to do?

I don’t often write about pesticides, plasticizers, heavy metals or other such potentially toxic substances in food because there usually isn’t enough science available to draw firm conclusions about how much of them is OK to consume.

At high concentrations they are demonstrably toxic.  But in food and water, they appear in amounts measured as parts per billion (ppb) or trillion, and it is difficult to know how harmful they may be at such levels.

The big question: is there a threshold for harm or are they unsafe at any level of intake?  The history of regulation of such substances is one of constant reduction in levels considered safe.

They derive in large part from industrial processes, and attempting to regulate them confronts large and powerful industries eager to argue that low levels are safe.

Now Consumer Reports has tested samples of juice and finds levels of arsenic  higher than allowed in drinking water:

  • 10% of the samples contained levels of arsenic that exceed EPA drinking-water standards of 10 ppb.
  • 25% contained levels of lead greater than the FDA’s 5 ppb standard for bottled water.
  • Most arsenic was inorganic, a form linked to cancer, cardiovascular disease, immune disorders, and type 2 diabetes.
  • 35% of children age 5 and younger drink juice in amounts higher than recommended by pediatricians.
  • No federal standards exist for amounts of lead and arsenic in juice.

This is a food systems issue.  Inorganic arsenic gets into food from soil contaminated with arsenic-containing herbicides and waste from animals and chickens fed arsenic-containing additives.

Consumer Reports says:

  • FDA should set a standard for total arsenic in juice at 3 ppb and 5 ppb for lead.
  • EPA should lower the 10 ppb drinking-water limit for arsenic.
  • Parents should limit juice servings to small children.

What does the FDA have to say?

The Food and Drug Administration has every confidence in the safety of apple juice…small amounts of arsenic can be found in certain food and beverage products—including fruit juices and juice concentrates….there is no evidence of any public health risk from drinking these juices…FDA has been testing them for years [see the FDA’s Q and A].

As if this were not enough to worry about, Food Quality News reports increasing concern about the amount of arsenic in rice, the dangers of such levels for pregnant women, and the need to establish better standards for safe levels of arsenic in foods.

Consumer Reports is especially concerned about a study published in the Proceedings of the National Academy of Sciences linking rice consumption to levels of arsenic in the urine of pregnant women.   It notes that children in the U.S. typically are fed rice cereal as their first solid food, making them especially vulnerable to the effects of arsenic.

Arsenic gets into rice in the same way that it gets into apples and grapes, but rice is especially efficient in taking up arsenic from soil.

I see all this as further evidence that agricultural practices are key determinants of public health and that we badly need:

  • More and better research on the effects of small amounts of arsenic, lead, pesticides, and other such toxins in food
  • Research on how to remove such toxins from soil
  • Federal safety standards for arsenic in foods and beverages; the Consumer Reports recommendations make sense
  • Regulations that restrict use of arsenic drugs in animal agriculture and of pesticides containing arsenic
  • Restrictions on the amount of juice and rice given to children

It’s great that Consumer Reports is doing this kind of research but federal agencies should be doing a lot more of it too.

Food Matters: front-of-package labels again

My monthly (first Sunday) Food Matters column in the San Francisco Chronicle appears today.  This time, it’s about the fuss over front-of-package labels.

Q: I’m completely confused by all of the little check marks and squares on food packages telling me they are healthy. Do they mean anything?

A: The Food and Drug Administration feels your pain. It sponsored two studies by the Institute of Medicine to rationalize front-of-package nutrition ranking systems.

The institute released its second report last month; it advises the FDA to allow front-of-package labels to state nothing but calories and nutrients to avoid: saturated and trans fat, sodium and sugar (go to sfg.ly/sUptQR).

The institute’s proposal gives products one point for not containing too much of each of these nutrients. It suggests displaying the points like Energy Stars on home appliances with zero to three stars, depending on how well the product meets nutritional criteria.

This is a simple system, instantly understandable. I think it is courageous. The institute’s proposal benefits consumers. It does not help companies sell junk food.

Selling or educating?

No food company wants to display nutrients to avoid. For the food industry, the entire point of front-of-package labels is to market products as healthy or “better for you” no matter what they contain. Front-of-package labels are a tool for selling, not buying. They make highly processed foods look healthier.

Will companies accept a voluntary labeling scheme that makes foods seem worse? Doubtful.

Nutrition ranking symbols began appearing on food packages in the mid-1990s, when the American Heart Association got companies to pay for displaying its HeartCheck.

Food companies then established their own systems for identifying “better-for-you” products. PepsiCo, for example, developed its own nutritional standards and proclaimed hundreds of its snacks and drinks as “Smart Choices Made Easy.”

In an attempt to bring order to this chaos, food companies banded together to develop an industry-wide system. Unfortunately, their joint Smart Choices checkmark appeared first on Froot Loops and other sugary cereals. The ensuing ridicule and legal challenges forced the program to be withdrawn.

At that point, the FDA, backed by Congress and other federal agencies, asked the Institute of Medicine for help.

The institute released its first report last year. It revealed inconsistencies in the 20 existing ranking schemes from private agencies, food companies and supermarket chains. Toasted oat cereal, for example, earned two stars in one system, a score of 84 (on a scale of 100) in another, and a score of 37 in a third.

The report said labels should display only calories and to-be-avoided nutrients. Labels should not display “good-for-you” nutrients – protein, fiber, and certain vitamins and minerals – because these would only confuse consumers and encourage companies to unnecessarily add nutrients to products for marketing purposes.

Although the FDA was waiting for the second institute report before taking action, the food industry wasted no time. The Grocery Manufacturers Association and Food Marketing Institute introduced their own system.

Complicated approach

They got their members to agree to a more complicated system, “Nutrition Keys,” based on nutrients to avoid but also including up to two “good-for-you” nutrients.

Food companies immediately put Nutrition Keys’ symbols – well established to be difficult for consumers to understand – on package labels where you can see them today. Now called Facts Up Front, the symbols are backed by a $50 million “public education” campaign.

The reasons for the industry’s preemptive strike are obvious. The second Institute of Medicine report gives examples of products that qualify for stars – toasted oat cereal, oatmeal, orange juice, peanut butter and canned tomatoes, among them.

It also lists the kinds of products that would not qualify for stars, including animal crackers, breakfast bars, sweetened yogurt and chocolate milk.

So the industry argues that consumers “want simple and easy to use information and should be trusted to make decisions for themselves and their families … rather than have government tell them what they should and should not eat.”

But why, you ask, does any of this matter? I view front-of-package labels as a test of the FDA’s authority to regulate and set limits on any kind of food industry behavior. If the FDA cannot insist that food labels help the public choose healthier foods, it means the public has little recourse against any kind of corporate power.

Perhaps Facts Up Front will arouse the interest of attorneys general – just as the Smart Choices program did.

In the meantime, the industry’s pre-emption of FDA labeling initiatives is evidence that voluntary schemes don’t work. Labeling rules need to be mandatory.

Let’s hope the FDA takes the Institute of Medicine’s advice and starts rule-making right away.
