Weekend reading: Michael Moss’s Hooked

Michael Moss.  Hooked: Food, Free Will,and How the Food Giants Exploit Our Addictions.  Random House, 2021.

This follows Michael Moss’s Salt Sugar Fat which was about how food companies used these ingredients to hook us on junk food.  The new book focuses on the “addictive” qualities of junk foods—what we are now calling “ultra-processed.”  I put addictive in quotes because his definition is looser than others I’ve seen: habits that are hard to quit.

By this definition, his book provides convincing evidence for what food companies do to make their products irresistible—remember Frito Lay’s “You can’t eat just one?”

The book starts by going into the physiology of addiction:

When we taste sugar, the taste buds on our tongue send the signal.  By contrast, the signal for fat gets transmitted by the trigeminal nerve that extends from the roof of the mouth to the brain.  Food that has both sugar and fat will activate these two different paths, sending to separate alerts, and thus doubling the arousal of a brain that appears to place a high value on information for information’s sake [62].

No wonder we like ice cream so much.

In speaking about how the food environment sets us up for overeating, he says:

…we simply haven’t had anywhere near the time we would need, vis-à-vis evolution, to catch up with the dramatic changes in food and our eating habits of the past forty years.  As a result, we are fundamentally mismatched to the food of today.  Small [Dana Small, an expert Moss interviewed] puts it this way: “It’s not so much that food is addictive, but rather that we by nature are drawn to eating, and the companies have changed the food [p. 99].

Moss is a terrific writer and tells a compelling story.  Even if you don’t have a problem resisting fast food, sodas, or chocolate, this book has a lot to say about why so many people have put on pounds during the Covid-19 pandemic.

More on the 2020 Dietary Guidelines

I only have a few more comments about the Dietary Guidelines beyond what I posted last week.

One is my surprise that the USDA did not do a new food guide.  The existing one, after all, dates from the Obama administration.  It has not changed.

Here’s how it is explained in the new guidelines:

My translation: Eat more plant foods, eat less meat, avoid ultraprocessed foods (including sugary beverages).

This requires a translation because the guidelines say nothing about ultraprocessed junk foods, and they try hard to avoid singling out foods to avoid.

These guidelines are similar to those in 2015 and are, therefore, woefully out of date.

They do mention the pandemic, once:

The importance of following the Dietary Guidelines across all life stages has been brought into focus even more with the emergence of COVID-19, as people living with diet-related chronic conditions and diseases are at an increased risk of severe illness from the novel coronavirus (p. 4).

They do mention food insecurity several times, for example:

In 2019, 10.5 percent of households were food insecure at least some time during the year. Food insecurity occurs when access to nutritionally adequate and safe food is limited or uncertain. Food insecurity can be temporary or persist over time, preventing individuals and families from following a healthy dietary pattern that aligns with the Dietary Guidelines. The prevalence of food insecurity typically rises during times of economic downturn as households experience greater hardship. Government and nongovernment nutrition assistance programs help alleviate food insecurity and play an essential role by providing food, meals, and educational resources so that participants can make healthy food choices within their budget (p. 50).

And they do mention food assistance programs (on page 81), although they do not discuss how the USDA has been relentless in trying to cut those programs.

Nothing about food systems.  Nothing about the effects of food production and consumption on climate change and sustainablity.

Nothing about eating less meat other than implying that eating less processed meat might be a good idea.

One other point: the complexity is increasing.  Here is the history of the page numbers:

As I’m fond of saying, Michael Pollan can do all this in seven words: “Eat food.  Not too much.  Mostly plants.”

If we can’t do better than this 164 pages of obfuscation, isn’t it about time to stop requiring these things every five years?

Using the pandemic as a business opportunity, European version

Food Safety News reports that the European Commission is getting increasingly upset about fraudulent claims that specific food and supplement products will boost immunity and help protect against Covid-19 or even cure it.

Alas, they will not.

The EC is worried about online advertisements.  Food Safety News reported more than 350 cases of such claims in June.  Now there are even more.

In the US, we mostly see this sort of thing—websites from the supplement industry telling you to take supplements.  Here  is what this one claims, with my comments in red.

Supplements can help you address nutrient insufficiencies or deficiencies in your diet—but can they help you fight COVID-19? Not as far as anyone knows.

While there hasn’t been specifically-targeted research to determine which—if any—nutrients should be FDA-approved to ward off the virus, [Indeed] supplements are an ideal way to keep your body and your immune system functioning at optimal levels. No, they are not.  Food works much better.

As a result, many physicians and other health and wellness experts recommend beginning a simple supplement routine to ensure your body has the nutrients it needs to stay healthy.  Many others do not, and neither do I.…The supplements you take during the COVID-19 pandemic may not be specifically developed to ward off the coronavirus. Right.  So don’t expect them to work.

Still, research has shown that they all play an important role in boosting the immune system, preventing respiratory damage, strengthening the body against viral infections, reducing inflammation—or all of the above.  This is true, but largely in experimental studies likely to have been funded by the supplement industry.

Obviously, I am not a fan of supplements.  There just isn’t much evidence that they do anything useful for healthy people, and healthy people are the ones most likely to be taking them.

With respect to Covid-19, the best preventive strategy is avoidance (masks, distancing, etc).

The best immune-boosting strategy is to eat a healthy diet–largely (but not necessarily exclusively) plant-based, balanced in calories, and with minimal amounts of ultra-processed junk foods.

And let’s all hope the vaccination comes soon and works like a charm.

New report: Big Food vs. Public Health During the Pandemic

Here’s a new must-read report:

This is a thorough and carefully done analysis of the ways in which Big Food companies took advantage of the Covid-19 pandemic as a marketing opportunity.  The report gives more than 40 specific examples of corporate:

Nutri-washing: Coupling “solidarity actions” with aggressive marketing of junk food and sugary drink brands, which helped polish corporate images
Positioning ultra-processed food and drinks as “essential products” when they are not healthy foods
Playing both sides: Carrying out philanthropic actions while actively lobbying against healthy food policies
Using charity to push junk food: Donating ultra-processed food and drinks to vulnerable populations

Here’s just one example:

The report is short and beautifully designed.  It comes from the Global Health Advocacy Incubator.  This group produces tools for advocacy, among other useful items.

As Bettina Siegel wrote earlier this year.

America’s poor diet is the leading cause of poor health and is responsible for more than half a million deaths per year. And if our current comfort food bender demonstrates anything, it’s that when people’s sense of security is fundamentally threatened, they’re very often compelled to seek relief and pleasure in unhealthy food.

The report shows how food companies take advantage of our current vulnerabilities.  That’s another reason why the UK’s stop-marketing proposal (I wrote about it yesterday) is so badly needed.

“Healthy” kids food: an oxymoron?

Foods marketed to kids are an enormously profitable enterprise, but most foods marketed to kids are ultraprocessed junk foods.  Companies are scrambling to come up with profitable food items for kids that might actually be good for them.  Hence: “healthy” kids food.

By the way, kids don’t need special foods designed for them; they are perfectly capable of eating anything that adults eat, cut or smashed to size and in smaller portions, of course.

General Mills is bringing back its classic recipes for four of its cereals: Cocoa Puffs, Golden Grahams, Cookie Crisp and Trix, according to a release. This reformulation back to retro ’80s recipes is permanent and is now available at retailers nationwide.

Just what kids do not need.  Sigh.

Industry-funded study of the week: Beyond Meat

Stanford University issued a press release to announce the results of a study comparing physiological effects of eating plant-based meat alternatives (Beyond Meat) to eating foods of animal origin.

A diet that includes an average of two servings of plant-based meat alternatives lowers some cardiovascular risk factors compared with a diet that instead includes the same amount of animal meat, Stanford Medicine scientists found.

The study:  A randomized crossover trial on the effect of plant-based compared with animal-based meat on trimethylamine-N-oxide and cardiovascular disease risk factors in generally healthy adults: Study With Appetizing Plantfood—Meat Eating Alternative Trial (SWAP-MEAT).  Crimarco A, et al.  American Journal of Clinical Nutrition, August 11, 2020.  https://doi.org/10.1093/ajcn/nqaa203

Overall conclusion: “This study found several beneficial effects and no adverse effects from the consumption of plant-based meats.”

The sponsor: “Supported by a research gift from Beyond Meat Inc. (to CDG)…Funding for this study was provided by Beyond Meat. In an effort to reduce any influences on the outcomes of this study, a statistical analysis plan was submitted to ct.gov. The main analysis was conducted by a third-party individual who had no involvement with the study design or collection of data, and was blinded to all study participants.

Comment

Ordinarily, I would simply present this study as a classic example of how industry-funded studies predictably produce results that favor the commercial interests of their sponsors, a topic to which I devoted my book, Unsavory Truth: How Food Companies Skew the Science of What We Eat.

But CDG is Christopher Gardner, the study’s lead scientist, whose impressive track record of managing complicated clinical trials of diet and health I greatly admire.

Gardner describes himself as a vegan (meaning that he eats no animal products).

Knowing of my concerns about industry-funded research, he wrote me some months ago to say that this study was in the works and to point out that he has done at least six industry-funded studies with null findings (he sent me a PowerPoint slide deck to prove it).  In his correspondence, he said:

  • “I believe this is the FIRST industry funded study I’ve run that had a significant positive health finding.”
  • “Beyond Meat was not involved in design or analysis, and to this day still doesn’t know the study outcome.”
  • “I’m preparing myself for being called out as a vegan industry shill….hoping I’ve established a reputation for objectivity to withstand this ?”
  • “PS – Hope you enjoy the study acronym (Study With Alternative Plantfood – Meat Eating Alternative Trial: SWAP-MEAT)” [Indeed I do].

OK.  So let’s take this study on its merits.

Gardner asked healthy non-vegetarian adults (36) to consume 2 servings a day of either Beyond Meat or regular meat (what the study calls Animal Meat).  The Beyond Meat and Animal Meat were provided to participants.  The rest of their diets was on their own.

For 8 weeks, they ate Beyond Meat or Animal Meat.  For the next 8 weeks, they switched over to the other kind.

Results: Participants consuming Beyond Meat displayed lower levels of

  • LDL-cholesterol (the bad kind)
  • Body weight (by 1 or 2 pounds)
  • TMAO (Trimethylamine N-oxide)—but only for those who consumed Animal Meat first and Beyond Meat second (not the other way around)

Beyond Meat may be plant-based, but it is ultraprocessed.  FoodNavigator produced a nice comparison.

Beyond Meat would dearly love to demonstrate that its ultraprocessed composition is immaterial to its health benefits.  Hence: this study.

Beyond Meat is already using it for marketing purposes: “New study finds health benefits of plant-based meats.”

As I see it, there are two issues here: (a) what else the participants were eating and (b) the significance of the TMAO measurements.

(a) The diet: This was not a controlled dietary intake trial conducted in a closed metabolic ward.  Participants were free to eat whatever they liked and how much they liked.  They lost a little weight during the Beyond Meat phase, which means they must have been eating fewer calories during that phase, as they reported (this graph is in the Supplementary material).

Reported daily calories were under 2000, which means that lots of calories must not have been reported.  So it’s hard to know what the weight loss is actually due to.

(b)  TMAO: You make TMAO after you eat foods containing choline, a compound common in animal-based foods: meat, poultry, fish, and eggs.  A 2019 editorial review in JAMA discusses the association of TMAO with heart disease risk.

Now, researchers are homing in on another possible culprit: a dietary metabolite linked to red meat called trimethylamine N-oxide, or TMAO. Three recent meta-analyses confirmed that high blood levels of TMAO are associated with increased risks of cardiovascular disease and all-cause mortality. One of the studies, published in the Journal of the American Heart Association in 2017, found a more than 60% heightened risk of both major adverse cardiovascular events and death from all causes in people with elevated TMAO. Other research has associated higher TMAO levels with heart failure and chronic kidney disease.

On the other hand, an analysis by Dr. Bret Scher raises questions about whether TMAO has any real meaning for health (and I thank Stephen Zwick for sending this to me).

In my opinion, this is an example of a well-run study that, in the end, lends very little to our knowledge of human health….The main outcome from this intervention had to do with TMAO. Why is this problematic? Well, it has to do with the fact that small, short-term trials are unable to measure meaningful endpoints, such as who lives, dies, or who gets heart disease.  So, instead, the authors have to choose the surrogate outcome markers that they believe relate to human health.

Dr. Scher believes that “There is no convincing evidence that these results impact someone’s health.”  As Scher has discussed previously, he sees no cause-and-effect relationship between TMAO levels and health.  You can read his arguments here:

The bottom line?  This study suggests that two servings a day of Beyond Meat is unlikely to be harmful.  Whether substituting Beyond Meat for real meat is truly useful for health in the absence of other dietary changes remains to be confirmed, hopefully by independently funded research.

Dietary Guidelines Advisory Committee releases report

The report of the 2020 Dietary Guidelines Advisory Committee is now available in online preprint.

It sets a record at 835 pages.

Its conclusions are highly consistent with those of previous Dietary Guidelines.

It recommends eating more of these foods:

Common characteristics of dietary patterns associated with positive health outcomes include higher intake of vegetables, fruits, legumes, whole grains, low- or nonfat dairy, lean meat and poultry, seafood, nuts, and unsaturated vegetable oils.

It recommends eating less of these foods:

The Committee found that negative (detrimental) health outcomes were associated with dietary patterns characterized by higher intake of red and processed meats, sugar-sweetened foods and beverages, and refined grains.

It retained the recommendation: Eat less red and processed meats

It retained the recommendation to eat less saturated fat (replace with polyunsaturated or monounsaturated)

Thus, the Committee recommends that dietary cholesterol and saturated fat intake be as low as possible within a healthy dietary pattern, and that saturated fat intake be limited to less than of 10 percent of total energy intake, as recommended by the 2015-2020 Dietary Guidelines for Americans. This recommendation applies to adults and children ages 2 years and older.

It tightened up restrictions on alcoholic beverages from 2 drinks a day for men to 1 drink:

The Committee concluded that no evidence exists to relax current Dietary Guidelines for Americans recommendations, and there is evidence to tighten them for men such that recommended limits for both men and women who drink would be 1 drink per day on days when alcohol is consumed.

It tightened up restrictions on added sugars, from 10% of calories to 6%:

After considering the scientific evidence for the potential health impacts of added sugars intake, along with findings from model-based estimations of energy available in the dietary pattern after meeting nutrient requirements, the Committee suggests that less than 6 percent of energy from added sugars is more consistent with a dietary pattern that is nutritionally adequate while avoiding excess energy intake from added sugars than is a pattern with less than 10 percent energy from added sugars.

What’s missing?

  • Salt: The report says remarkably little about sodium beyond that it is overconsumed and people should “reduce sodium intake.”  It’s possible that I missed it, but I could not find suggestions for quantitative limits.
  • Ultraprocessed: The word does not appear in the report except in the references.  A large body of evidence supports an association of ultraprocessed foods to poor health.  If the committee considered this evidence, it did not spell it out explicitly.
  • Sustainability: This was off the table from the beginning but this committee recommends that it be considered next time in the context of a food systems approach to the Dietary Guidelines (p.771).

Comment

This is an impressive, solid, conservative review of the existing science highly consistent with previous Dietary Guidelines but with mostly stronger recommendations.

This committee was up against:

  • A tight time frame
  • A first-time mandate to review literature on infancy, pregnancy, and lactation in addition to that for adults
  • A first-time process in which the agencies set the research agenda, not the committee
  • The Coronavirus pandemic

At the outset, I was concerned that the committee members might be heavily biased in favor of food industry interests.  If they were, such biases do not show up in the final report.  I think this committee deserves much praise for producing a report of this quality under these circumstances.

Want to weigh in on it? 

The agencies are taking public comments until August 13.  On August 11, there will be an online public meeting for even more comments.

What’s next?

This report is advisory, only.  USDA and HHS must boil this down to the actual 2020 Dietary Guidelines.  Whereas the committee process was transparent, the boiling down process is highly secretive, or was in 2015.  It will be interesting to see what the agencies do, especially given the heavy lobbying by proponents of meat, saturated fat, and low-carbohydrate diets.

Weekend reading: Eat Like the Animals

David Raubenheimer and Stephen J. Simpson.  Eat Like the Animals: What Nature Teaches Us About the Science of Healthy Eating.  Houghton Mifflin Harcourt, 2020.

I did a blurb for this book:

Eat Like the Animals is a must-read. This beautifully written book proposes a highly original and compelling explanation for why so many of us gain weight in today’s overprocessed food environment. Raubenheimer and Simpson are biologists who use their deep knowledge of animal and insect physiology, evolution, and feeding behavior to construct a compelling hypothesis: we share with animals an innate appetite for protein that regulates what we eat.

Here is one sample:

Our analyses, inspired all those years ago by experiments with locusts and then extended to primates in the wild, had provided a radical new insight into the obesity epidemic. Ultraprocessed foods make us fat not because we have such strong appetites for the fats and carbs they contain, as is often thought to be the case. Rather, we become overweight because our appetite for protein is stronger than our ability to limit fat and carb intake. So, when protein is diluted by fats and carbs, as it is in ultraprocessed foods, our appetite for it overwhelms the mechanisms that normally would tell us to stop eating fats and carbs. As a result, we eat more than we should, more than is good for us.”  (p. 144)

And here is another:

That realization provided one of the key insights of this book and one that has guided us ever since: the strong appetite for protein shared by all animals can lead us to eat too much or too little of other nutrients, including fats and carbs. If our protein appetite is not satisfied, we will overeat the other two. Once we get enough protein, our appetites cease driving us to eat more. That’s as simple as we can make nutrition—without oversimplifying it.  (P. 180)
