Do we have a food movement? The New York Times food issue

The New York Time published its annual food issue on Sunday, this one with the theme, “Can Big Food Change?”

In the circles in which I travel, Michael Pollan’s “Big food strikes back: Why did the Obamas fail to take on corporate agriculture?” caused the biggest stir.  Here’s what set people off:

On “Outlobbied and Outgunned:”  The word I’ve been using to describe food industry lobbying against Michelle Obama’s Let’s Move! campaign is ferocious.

I’ve always thought that Mrs. Obama must have picked the goal of Let’s Move!—“Ending childhood obesity in a generation“—as a safe, bipartisan issue that Republicans and Democrats could all get behind.  Doesn’t everyone want kids to be healthy?

I can’t imagine that she could have predicted how controversial matters like healthy school lunches or nutrition standards for food advertising to kids would become.

Whatever.  The food industry’s response to everything Let’s Move! tried to do was ferocious.

Despite all that, as I’ve said, Let’s Move! managed to accomplish some important gains: healthier school meals, more informative food and menu labels, the White House garden, and—most important—getting food issues on the national agenda.

On “the food movement barely exists:” I once taught a course on food as a social movement with Troy Duster, a sociologist then at NYU, who had much experience teaching about social movements.

He made one point repeatedly: those who are in the middle of a social movement cannot possibly judge its effectiveness.  You can only know when a movement has succeeded or failed when it is over.

This one is not over yet.

This movement, fragmented in issues and groups as it most definitely is, may not have clout in Washington, DC, but it is having an enormous effect on supermarkets, food product manufacturers, fast food chains, the producers of meat, eggs, and poultry, and young people in this country.

How else to explain:

  • The vast improvement in the quality of foods sold in supermarkets
  • The rush of food product makers to remove artificial colors, flavors, trans-fats. and other potentially harmful food additives, including sugars and salt
  • The insistence of fast food chains on sourcing meat from animals raised without hormones or antibiotics
  • The actions of meat, egg, and poultry producers to care for their animals more humanely
  • Soda tax initiatives in so many cities

And my personal favorite,

  • The enormous numbers of college students clamoring for courses about food systems and the role of food in matters as diverse as global resource inequities and climate change.

As Troy Duster kept telling us, it’s not over until it’s over.

While waiting for enlightenment, let’s celebrate the proliferation of food organizations.  They are all working on important issues and doing plenty of good.

And yes, let’s encourage all of them to move beyond the local, engage in national politics, and put some pressure on Washington to come up with better food policies.

My Time Magazine op-ed: We are still confused about calories

While I was in Israel last week, Time Magazine published an op-ed it had invited me to write.  Here it is.

Even in ‘healthy’ foods, calories can tick up fast

Nutrition professor that I am, if I could teach just one thing to the American public, it is this: Larger portions have more calories.

Please don’t laugh.

If we all understood this, the United States would not have an obesity problem. And the recent revelation that places like Chipotle and Panera serve meals with more calories than McDonald’s would surprise nobody. We would realize the former’s “health” aura blinds people. But this basic rule applies to those places, too.

We would also, strangely, thank McDonald’s for at least one thing: It limits portion sizes. It’s easy to pile on calories by asking for a little more of this, then a refill of that—without realizing that the calories surpass those in McDonald’s meals.

It’s not easy to understand calories. They are not intuitive because they’re abstract. They cannot be seen, tasted or smelled, and are extraordinarily difficult to count accurately, even for scientists. My colleague Lisa Young, author of The Portion Teller, once asked an entry-level nutrition class to guess the calories in 8-ounce and 64-ounce soft drinks. We didn’t expect students to know that an 8-ounce Coke has 100 calories—but we certainly expected them to multiply whatever they guessed by eight for the 64-ounce drink. But the average multiplier was three.

The unintuitive relationship between amounts eaten and calorie intake is one of the reasons behind public health campaigns like the new soda tax in Philadelphia, which in part aims to get people to think twice before drinking sugary beverages. No nutritionist worries about an occasional 90-calorie mini-can of soda. But many people drink sugary beverages in 12-ounce, 20-ounce or liter sizes. Bigger sizes mean more calories, and more calories means more weight unless you compensate with physical activity.

But exercising off calories takes a lot of work. That was the message of aNew York City health department campaign demonstrating the need to walk the three miles from Union Square to Brooklyn to compensate for the nearly 300 calories in a 20-ounce soft drink.

And let’s not forget that all of the calories in soft drinks come from sugars, which provide nothing but calories—no vitamins, no minerals, no fiber. They may be delicious, but they have no redeeming nutritional value. That’s why their calories get called “empty.”

Does where calories come from make any difference? Yes, but in complicated ways. Weight balance depends on calories. But managing weight—and overall health—very much depends on where those calories come from. It’s not so easy to overeat vegetables, fruits and whole grains, or relatively unprocessed foods of any kind, because these tend to be bulky and fill you up before you eat too much. You can still gain weight on these healthy foods, but it’s harder. Chipotle may have healthier calories than McDonald’s, but its calories still count.

Sugars post particular problems because they induce insulin production, but also because they make foods taste good. We eat something sweet and want more of it. We start loving sweet tastes in infancy. Breast milk is sweet; it contains a tablespoon of lactose per cup and that’s there for a reason—to make babies do the work of nursing.

But sodas are very sweet. A 12-ounce soda contains more than threetablespoons of sugars—and the calories that go with them.

It’s also very hard to separate the effects of sugars from their calories. If scientists want to know whether the calories from sugars are worse than those from any other foods, they need to feed people precisely the same number of calories from diets widely varying in their content of sugar, other carbohydrates, protein, and fat for a long enough period of time to get meaningful results. The only way to do something like this would be to keep the study subjects under lock and key for as long as it takes—inconvenient, to say the least, and very expensive.

Until the science is resolved, we can all agree that eating less sugar is a good idea for just about everyone. Sugars are nutritionally empty, are hidden in foods, and encourage overeating.

Notice that I said less, not none. Personally, I love desserts and would never want to give up sugars entirely or ask anyone else to do so. I just follow my own dietary advice: eat lots of vegetables and other relatively unprocessed “real” foods, and for everything else, pay attention to portion size. This way, an occasional sugary treat is a pleasure and nothing to worry about.

Food politics: Mexico then and now

I’m in Mexico City doing talks for El Poder del Consumidor, the advocacy group in part responsible for Mexico’s soda tax.  I had some time to be a tourist yesterday afternoon and got to see the Diego Rivera murals at the Palacio Nacional.

These are enormous, and stunning.  They deal with the history of Mexico in conflict and in peace.  Look closely, and you see Rivera’s deep respect for Mexico’s traditional food culture.

Along the corridor flanking the main mural, for example, is a painting above a plaque listing what the world owes Mexico—corn, obviously—but also beans, tobacco (oops), chocolate, hemp, and tomatoes.

San Francisco State vs. Pouring Rights Contracts

When I was in San Francisco last week, I met Janna Cordeiro and Real Food Challenge students from San Francisco State University (SFSU) who are taking on Big Soda.  As Janna explained in an e-mail,

Last spring, SFSU administration quietly released an RFP to solicit a corporate sponsor for Pouring Rights.

The Pouring Rights contract —for a 1 time minimum $2 Million donation and yearly $125K donation— not only includes 80% access to all drinks sold on campus, naming rights for the sports stadiums (and scholarships, seats, etc), access to students and alum for social media campaigns, access to STUDENT-owned campus center, and on and on BUT also an endowed chair in the school of the sponsor’s choice. Pepsi Professor anyone?

She points out that “The students believe that the release of the RFP violated important shared governance agreements that guide the campus, and that it was intentionally kept very low profile.”

She also notes that since San Francisco’s soda tax campaign, several high profile policies limiting SSBs have been passed:

1) City of SF has passed 2 important pieces of legislation: warning label requirements on ads,  ban on use of city funds to purchase ssbs (including the many contractors such as Department of Children Youth and Families), and ban on sub ads on city property

2) The SFUSD passed a comprehensive wellness policy which bans all sugary drinks sold or offered on school grounds including fundraisers/festivals/ and staff/teachers drinking.

3) Most major hospitals are SSB free INCLUDING all of UCSF campuses and our public hospital.

This means:

Essentially, SFSU and our City College Campuses are the only public spaces where SSBs are sold or advertised. So, we can’t let the SFSU administration go through with this, and this group of students from the Real Food Challenge SFSU are stepping up to fight back. Let’s join them and show our support!  They have already organized demonstrations when PepsiCo and Coca-Cola were on campus for their presentations, but have much more planned. The also have a 15 page resolution that covers ALL the bases!

And here’s her call to action:

Two reports: Who is Obese? How to Curb Global Sugar?

The first report is from the UK.   Fat Chance? Exploring the Evidence on Who Becomes Obese is a curious example of what happens when a sugar company (AB Sugar) partners with a health organization (2020 Health) to produce a policy document.

The report examines the role of age, gender, socioeconomic factors, the built environment, mental health and disability, sleep, bullying and child abuse, smoking, ethnicity, and religion as factors in obesity—everything except diet and activity levels.

The press release for the report gives key findings, among them:

  • Obesity rates are rising rapidly among the poor as well as other groups who experience social instability.
  • Uncertainty seems to be a significant factor for weight gain.
  • Fast food outlets near working environments have a significant impact on the BMI of men; the lack of green space has an impact on obesity rates particularly among girls.
  • Half of all people suffering with psychosis are obese.
  • Parental obesity, especially in mothers, is a far more predictive factor in childhood obesity than is ethnicity.

Its authors write:

What is particularly highlighted in recent research, though rarely explicitly stated, is that obesity rates seem to be deeply influenced by social change (not just influences within static social categories). The studies we have compiled for this review show a subtle trend that has become increasingly evident over the last decade. It is highlighted in economic mobility, rising rates of mental illness, technological habits and engagements, and rapidly shifting urban ground. Argued here, broadly speaking, is that many of these categories strongly hint to a meta-structure that remains profoundly under-researched and largely ignored. This is the structure of uncertainty, a type of habitus that influences the terms of emotional engagement between an individual and their daily life. Insidiously, it undermines health seeking behaviour by making daily decision processes cognitively intolerable and emotionally taxing.

They conclude:

…approaches to obesity that recognise and incorporate complexity might impact a host of rising health problems that affect communities across Britain. The same interventions that encourage healthy BMI may improve energy levels through metabolic process and sleeping habits, while reducing risk of mental health problems, diabetes and a range of other comorbidities not discussed in this report.

But they don’t say what those interventions might be.

Could they possibly have something to do with removing sugary drinks and foods from local environments?

For doing just that, the World Cancer Research Fund International has produced Curbing Global Sugar Consumption: Effective Food Policy Actions to Help Promote Healthy Diets & Tackle Obesity.

Examples of actions which have had these effects include school nutrition standards in Queensland, Australia; a vending machine ban in France; a front-of-package symbol that led to product reformulation in the Netherlands; soda taxes in France and Mexico; a programme targeting retail environments in New York City, USA; a programme promoting increased water consumption in schools in Hungary; school fruit and vegetable programmes in Netherlands and Norway; a healthy marketing campaign in Los Angeles County, USA and a comprehensive nutrition and health programme in France.

The first report asks us to solve problems of poverty, instability, and mental health before taking action to prevent obesity, even when actions are known to be effective.  The second calls for such actions now.

Could AB Sugar’s sponsorship possibly have something to do with this difference?

Sugar politics: a roundup of recent events

While I was visiting Cuba, formerly the largest supplier of sugar to the United States and now blocked from selling anything to us, I missed several stories about sugar.  It’s time to catch up with them.

On this last item the Post explains:

While other crop subsidies have withered, Washington’s taste for sugar has been constant. The sugar program, which has existed in various forms since the 1930s, uses an elaborate system of import quotas, price floors and taxpayer-backed loans to prop up domestic growers, which number fewer than 4,500.

Sugar’s protected status is largely explained by the sophistication and clout of a small but wealthy interest group that includes beet farmers in the Upper Midwest, cane growers in the South and the politically connected Fanjul family of Florida, who control a substantial part of the world sugar market.

Attempts to get rid of the sugar program have been constant, at least since the 1970s when I first started teaching about it, but to no avail.  Why not?  Because outrageous as the program is, it only costs the average American $10 per year—not enough to generate widespread opposition, apparently.

The bottom line on all this: eating less sugar is always a good idea.

Interview with Columbia University Public Health Students

For tonight’s Grand Rounds at Columbia University, I did an interview with FPOP (Food Policy and Obesity Prevention).

Food Policy Expert Marion Nestle on the Heinz-Kraft Deal, GMOs, and the Secret Ingredients to Healthy School Lunches

March 31, 2014—Years before the Reagan Administration decreed that ketchup was a vegetable, Marion Nestle saw the connections between the dinner table and politics. Nestle, the nation’s leading advocate for good nutrition, will address the Mailman School in a Grand Rounds talk tomorrow and kick-off Public Health Fights Obesity, a month-long series of lectures and special events, including an April 16 symposium on preventing childhood obesity.

Nestle, a professor and founder of the department of Nutrition, Food Studies, and Public Health at New York University, is the author of acclaimed books, includingFood Politics: How the Food Industry Influences Nutrition and Health, and most recently, Why Calories Count: From Science to Politics.

In anticipation of her Grand Rounds talk, the student group Food Policy and Obesity Prevention interviewed Nestle about everything from attempts to regulate Big Soda, GMO labeling, to school lunches done right.

The federal Dietary Guideline Advisory Committee recently published recommendations that for the first time considered issues of food sustainability. There has been a lot of controversy.    

The guidelines have always been controversial, but never anything like this. I think this is an example of how worried the food industry is about the pushback about diet and health in America. Sustainability is the “S word” in Washington. The guidelines committee is trying to do is what I’ve been advocating for a very long time, which is to bring agricultural policy in line with health policy. Right now the policies are completely divorced.

At the same time you have Heinz and Kraft joining forces.  

The food industry is in a defensive position because food and health advocates have been enormously successful in changing the market and changing people’s views. The fastest growing segment of the food industry is organics. The makers of processed foods are in retreat.  Warren Buffett must think there’s plenty of money to be made in selling junk foods.  I hope he’s wrong.

Is Big Food increasingly eyeing opportunities overseas?

If you can’t sell it here, you sell it there. The best example of this is the soda industry, which is the subject of my next book. There has been a 10- to 15-year decline in sales of carbonated sweetened beverages in the United States. It’s one of the great successes of health advocacy. To compensate, Coke and Pepsi are increasingly focusing their efforts overseas. Expect obesity and its consequences to follow.

Speaking of global commerce, should we be concerned about trade agreements like the Transpacific Partnership?

Food and Water Watch called it “NAFTA on steroids.” It’s very hard to know what’s going on because the negotiations are being done in secret. People are worried that a lot of the protections we have against bad things in food will be taken away on the basis of violations of trade agreements.

Closer to home, here in New York we’ve heard a lot about attempts to legislate on soda with failed attempt to limit portion sizes. Other areas have had more luck—

Not luck—skill! The only place in the United States where a soda tax has been successful is Berkeley. They did everything about advocacy right. Instead of framing it as a health argument, they framed it as an argument against corporate power: Berkeley versus Big Soda. And there was an enormous grassroots effort to engage the entire community. Community organizing is classic public health. Nobody does it very often. But when it’s done, it works!

Another issue people have been talking about is GMO labeling. 

I was on the FDA food advisory committee in 1994 when they were in the process of approving GMOs. Those of us who were consumer representatives told the FDA that it had to require labeling. I’m surprised it’s taken this long for there to be a major national uproar. From the beginning, the question was: if they don’t want labels, what are companies like Monsanto trying to hide?

Speaking of Monsanto, there was news this week that a chemical in their Round Up herbicide is a likely a carcinogen.

RoundUp also induces weed resistance, which has become an enormous problem for the industry. And most of it is used on GMOs. It’s a plant poison! Why would anyone think it would be good for health?

Are GMOs always bad?

The papaya that’s engineered to resist ring spot seems like a reasonable use of biotechnology to me. It saved the Hawaiian papaya industry. That’s the only example I can think of that’s beneficial. Most of the technology has been applied to commodity crops.

What about food insecurity? Can GMOs help?

If you want to help food-insecure nations, you need to empower them to do their own agriculture. That agriculture needs to be sustainable. GMO crops are not sustainable.  They require seeds, fertilizer, and pesticides, every year.


President Obama signs the Healthy Hunger-Free Kids Act in 2010

According to a new Rudd Center study, more kids are eating fruit at school. At the same time, there’s a lot of pushback against healthy foods at school.

In 2010, Congress passed the Healthy Hunger-Free Kids Act. That was bipartisan. Today, bipartisan seems out of the question. The Republicans want to roll the Act back. There’s no question it’s working in most schools that have people committed to it. There are huge advances being made in school food that carry over to food outside school. Kids come home and they want different foods because they see that eating healthy foods is valued.

How much is this change tied to school leaders compared to funding?

More funding would help. But some of the poorest schools have cafeterias where you walk in and the food smells good. They’re making it happen by cooking onsite with USDA commodity foods, which are unprocessed and cheap. Someone who knows how to cook can turn USDA surpluses into good meals. But not every school does that. I’ve been in schools where the food was terrible, the kids weren’t eating it, and the plate-waste was astronomical.  If the food service workers know the names of the kids, it’s a good sign the food will be good too.

For students interested in food and health, what sectors offer the most opportunity? Government? Nonprofit?

It depends on what you like. We need good people in government. It’s really important to have public health professionals work from within to make agencies like the FDA and Department of Agriculture do useful work. Everybody loves NGOs. It doesn’t matter which. Just do it!
