Agriculture policy needs to support health policy: Fruits and vegetables!

The Union of Concerned Scientists (UCS) released a new report yesterday: “The $11 trillion reward: How simple dietary changes can save lives and money, and how we get there.”

Never mind the hype ($11 trillion?  That’s too big to understand).   Whatever the real number, the report makes one thing clear: if we don’t get healthier, health care costs will rise.  A lot.

To stay healthy, we need to eat more fruits and vegetables (F&V).  But that’s not  so easy.  They are relatively expensive, not always easy to deal with, and thoroughly unsupported by federal agricultural policy.

To fix that, UCS calls for federal policies to:

  • Increase research on F&V.
  • Remove planting restrictions that stop commodity farmers from growing F&V.
  • Make crop insurance available for F&V producers.
  • Make healthy, locally grown food more available and accessible.
  • Promote the growth of farmers markets, local food outlets.
  • Facilitate the use of SNAP benefits at local food markets.
  • Educate consumers about F&V and how to prepare them.

Good news: FDA issues rules for a gluten-free claim on food packages

In what will surely be good news to people with celiac disease and other problems related to gluten intolerance, the FDA has just issued a final rule for defining foods as “gluten-free.”

This, after nine years of work on this issue (see timeline below).

The rule states that companies can label their products gluten-free if they contain less than 20 ppm (parts per million) gluten.  This, says FDA,

is the lowest level that can be consistently detected in foods using valid scientific analytical tools. Also, most people with celiac disease can tolerate foods with very small amounts of gluten. This level is consistent with those set by other countries and international bodies that set food safety standards.

As for gluten intolerance: Roughly 1% of the U.S. population has diagnosable gluten intolerance.  For those who do, eating grains containing gluten (wheat and some others) triggers an abnormal immune response that damages the intestinal tract.  To prevent symptoms—of which there can be many, none of them pleasant—people with this condition must scrupulously avoid eating gluten-containing foods.

The new rule will help establish some uniformity in labeling.  Note: gluten-free does not mean sugar-free.

In reading through the FDA’s notice in the Federal Register, I am struck by the complexity of the agency’s processes for doing something like this.

Policy wonk that I am, I immediately made a timeline.  This indicates that the FDA spent nine years getting to this point. Impressive, no?

World Health Organization takes on the food industry

I’ve just been sent a copy of  the opening address given by the Director-General of the World Health Organization, Dr Margaret Chan, to a Global Conference on Health Promotion in Helsinki on June 10.

Here is an excerpt from her extraordinary remarks:

Today, getting people to lead healthy lifestyles and adopt healthy behaviours faces opposition from forces that are not so friendly.  Not at all.

Efforts to prevent noncommunicable [chronic] diseases go against the business interests of powerful economic operators.

In my view, this is one of the biggest challenges facing health promotion…it is not just Big Tobacco anymore.  Public health must also contend with Big Food, Big Soda,and Big Alcohol.

All of these industries fear regulation, and protect themselves by using the same tactics.

Research has documented these tactics well. They include front groups, lobbies, promises of self-regulation, lawsuits, and industry-funded research that confuses the evidence and keeps the public in doubt.

Tactics also include gifts, grants, and contributions to worthy causes that cast these industries as respectable corporate citizens in the eyes of politicians and the public.

They include arguments that place the responsibility for harm to health on individuals, and portray government actions as interference in personal liberties and free choice.

This is formidable opposition. Market power readily translates into political power…

Not one single country has managed to turn around its obesity epidemic in all age groups.  This is not a failure of individual will-power. This is a failure of political will to take on big business…

I am deeply concerned by two recent trends.

The first relates to trade agreements. Governments introducing measures to protect the health of their citizens are being taken to court, and challenged in litigation. This is dangerous.

The second is efforts by industry to shape the public health policies and strategies that affect their products. When industry is involved in policy-making, rest assured that the most effective control measures will be downplayed or left out entirely. This, too, is well documented, and dangerous.

In the view of WHO, the formulation of health policies must be protected from distortion by commercial or vested interests.

Dr. Chan was courageous to say this so clearly.  Would that our health officials would be as brave.

The endless debates about salt: Don’t worry. Eat (real) food

Since 1980, U.S. dietary guidelines have advised eating less sodium (salt is 40% sodium, 60% chloride).  Although sodium is an essential nutrient, most Americans consume way more than they need or is good for them—around 3,400 milligrams a day.

The 2010 guidelines advised healthy people to consume no more than 2,300 mg per day (~6 grams, or 1.5 teaspoons).  They advised even less, 1,500 mg, for people with or at high risk for high blood pressure.  Since blood pressure increases with age in countries with high salt intake, this applies or will apply to just about everyone.  

In 2011, the Institute of Medicine said it was imperative to find effective strategies to lower salt intake.  This means dealing with processed and restaurant foods, because that’s where most of the salt comes from, as can be seen from this list of major food sources

Because consumers have no choice about the amount of salt in processed and restaurant foods, education cannot be enough to achieve salt reduction.  Scientists in Australia have just proved this point.

As I explained to a reporter,

Why anyone would think that nutrition education alone would change behavior is beyond me. By this time everyone should know that to change behavior requires not only education, but a food environment—social, political, economic—that supports and promotes the behavior change.

Most dietary sodium comes from processed foods, restaurant foods, and other pre-prepared foods.  All the label can do is say ‘don’t eat me’ It can’t help with what people can eat.

The easiest and most effective way to help people reduce sodium intake is to require food producers and food preparers to use less of it. Good luck with that. I’m not optimistic, particularly given the conflicting and confusing science. 

Ah yes.  The conflicting science.  The IOM now says that there’s no evidence one way or the other that reducing sodium below 2,300 mg per day, or even to 1,500 per day, does much good, and that low sodium intakes could be harmful (but this too is controversial).

Yes, they could, but as Mark Bittman blogs,    

It may be true that there are no benefits in an ultra-low-salt diet, but almost no one is eating an ultra-low-salt diet. It’s not quite like worrying about whether we get “enough” sugar, but it’s nearly as ridiculous.

And now, as Food Navigator explains, the IOM committee is complaining that its report has been badly misinterpreted.  All they said was:

As to whether we should cut back to 1,500 mg or to 2,300 mg sodium a day, meanwhile, the jury is out, says the IOM, not because consuming 1500 mg/day is dangerous, but because there is just not enough data on the benefits of consuming such low levels to support a firm conclusion.

IOM committee members were so bothered by misleading press accounts that they wrote an op-ed to JAMA to clarify:

Rather than focusing on disagreements about specific targets that currently affect less than 10% of the US population (ie, sodium intake of <2300 mg/d vs <1500 mg/d),  the IOM, AHA, WHO, and DGA are congruent in suggesting that excess sodium intake should be reduced, and this is likely to have significant public health effects. Accomplishing such a reduction will require efforts to decrease sodium in the food environment….

The bottom line, Bittman says (and I enthusiastically agree), is that

Salt intake — like weight, and body mass index — is a convenient baseline for public policy people to talk about. If you focus on eating less salt — and, indeed, less sugar — you will inevitably eat less processed food, fast food, junk food (it’s all the same thing.) If you eat less processed food (etc.) you eat more real food. If you eat more real food, not only are you healthier, but you probably don’t have to pay attention to how much salt you’re eating. Wowie zowie.

Attention policy wonks! It’s farm bill time again.

The Senate and House released their versions of the farm bill last week.  By size (1102 v. 576 pages) and extent of budget cuts ($23 billion v. $40 billion), these are  incompatible. I’m guessing that getting them passed and reconciled will require major compromises—hard to imagine for this dysfunctional Congress.

The Congressional Budget Office, according to the Hagstrom Report, estimates that the Senate bill will cost $955 billion from 2014 to 2023, and the House bill will cost $940 billion—but roughly $100 billion a year for the next 10 years.   Much is at stake.

The Union of Concerned Scientists (UCS), ever optimistic, has produced a report, The Healthy Farm: A Vision for U.S. Agriculture, identifying ways that the farm bill could—if there were any political will—support an agricultural system focused on producing abundant, affordable, and healthy food and on protecting the environment (also see its interactive healthy farm and take action sites).

During the coming days, I’ll take a stab at interpreting key pieces of the proposed bills.  Stay tuned.

2:00 p.m. addition: Jerry Hagstrom says the Senate Agriculture Committee has approved the farm bill by a vote of 15 to 5. Senators Roberts, McConnell, Johanns, Thune and Gillibrand voted no.  OK.  Now let’s see what the House does tomorrow.

Parke Wilde’s Food Policy is now out

For those of us who teach food policy and politics, a new textbook is most welcome, especially when it comes from Parke Wilde.  Wilde is now a professor at Tufts and a food policy blogger, but I first met him years ago when he was a reporter for the Community Nutrition Institute’s Nutrition Week.

His first book has just been released.

Marketing foods and drinks to kids in school goes on and on

I’ve just been sent a new report on the current status of marketing foods and beverages to children at school: Promoting Consumption at School: Health Threats Associated with Schoolhouse Commercialism.

This reportfrom the National Education Policy Center at University of Colorado, Boulder,  makes sobering reading.

As the press release explains,

In their quest for additional funding, many schools and school districts have allowed corporations to promote the consumption of sweetened beverages and foods of little or no nutritional value in school and in conjunction with school projects…corporations can seem philanthropic when they provide sponsored educational materials…to schools and teachers. These materials can be colorful and engaging, and may align with state and now Common Core standards, but they also present a worldview consistent with that of the sponsor.

If you think that the food companies are making good on their pledges to reduce marketing to kids, this report will make you think again.

Here are a few snippets:

  • Available data suggest that the total amount of money spent on advertising food and beverages to children, both in and out of schools, has decreased over the past few years.  However, any reduction in spending reflects at least in part a shift to less expensive, but more effective, alternative media advertising.
  • Food and beverage companies advertise in schools in multiple ways: (1) appropriation of space on school property, (2) exclusive agreements, (3) sponsorship of school programs, (4) sponsorship of supplementary educational materials, (5) digital marketing, (6) sponsorship of incentive programs, and (7) fundraising.
  • Teaching materials may not mention the sponsor but reflect the sponsor’s views, such as that all beverages count toward hydration.
  • Digital marketing to school kids is a deliberate strategy, as explained by a Coca-Cola executive:  “We’re especially targeting a teen or young adult audience. They’re always on their mobile phones and they spend an inordinate amount of time on the Internet.”
  • Health and wellness initiatives designed to promote physical activity and movement may appear to meet federal guidelines but “are problematic in that they shift the onus for obesity from the corporation’s responsibility to market healthy food to the consumer’s responsibility for making healthy choices.”

The report is a terrific summary of what’s happening with food marketing in schools, loaded with facts, figures, and references.  

In light of the evidence it provides, the report’s recommendation seems grossly understated:

Policymakers should prohibit advertising in schools unless the school provides compelling evidence that their intended advertising program causes no harm to children.

What’s missing from this report is a blueprint for action.

For that, you must go elsewhere, for example, to the Center for Science in the Public Interest, the Berkeley Media Studies Group, or the Rudd Center for Food Policy and Obesity.

USDA proposes rules for “competitive” snack foods

At long last, the USDA announced that it has released its proposed rules governing the nutritional content of snacks, sodas, and meals sold in competition with federally subsidized school breakfasts and lunches.

As soon as the rules get published in the Federal Register, which is supposed to happen this week, people will have 60 days to file comments.  Although USDA has not said when it will issue final rules, it did say that it will give schools another year to implement them.

The rules apply to foods sold outside the school meals in vending machines and a la carte lines.  They will not apply to fundraisers.  They set minimum standards.  States and localities that want stricter standards may do so.  A recent CDC analysis says states are already doing this (see Competitive Foods and Beverages in U.S. Schools: A State Policy Analysis).

Under the proposed rules, schools must provide:

  • Potable water at no charge [this alone is cause for celebration].
  • Real foods that are either something recognizable as a food or something that naturally contains 10% of the Daily Value in calcium, potassium, vitamin D, or fiber.
  • Snacks with less than 200 mg sodium per serving.
  • Desserts with less than 35% of calories from sugars or less than 35% of weight as sugars.
  • Beverages with no more than 40 or 50 calories per 8-ounce serving.

There are plenty of exceptions.   I can only guess that the exemption for sweetened yogurt—30 grams of sugars in 8 ounces—has something to do with dairy lobbying.

My immediate reaction: these rules are a big improvement and deserve much support.
